      • Baby Room (1 to 19-months)The lids of sensory bottles filled with pebbles were not securely fastened.The operator promptly removed the bottles from the room. Please glue the lids onto the sensory bottles to prevent potential choking hazards.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Push pins were observed being used on bulletin boards in rooms for 2 to 3-year-olds and 3 to 4-year-olds.Please ensure push pins are not used on bulletin boards and replaced with child safe alternatives to prevent potential choking hazards or accidental punctures.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Cloth towels were used in place of single-use paper towels. The inspectors were informed that the cloth towels were reused. Cloth towels may only be used once and then laundered. Cloth towels were observed to be used in the following areas:- The handwash station located in the 19-month to 2-year-old room- The handwash station located in the diapering area of the 19-month to 2-year-old room- The staff washroom on the top floor- The handwash station located in the 4 to 5-year-old roomThe operator was instructed to refill the paper towel dispensers. Please ensure that cloth towels are used once and then laundered. Diapering areas must be equipped with single-use paper towels for handwashing and cleaning to prevent cross-contamination. Ensure proper procedures are in place to guarantee that cloth towels are used only once before being laundered, or switch to single-use paper towels to maintain hygiene standards.

      • Baby Room (1 to 19-months)A cloth bean bag was observed without a removable cover for laundering.Please ensure the bean bag is equipped with a cover that can be removed for regular cleaning.

      • 19-month to 2-year-old room1) The carpets were observed to be soiled with debris and stains.Please deep clean the carpets located in this room.2) Toys were observed to be left in the sandbox. Please ensure all toys are removed, and cleaned and sanitized after use, especially in areas used by the 19-month to 2-year-old age group.3) A cloth bean bag was observed without a removable cover for laundering.Please ensure the bean bag is equipped with a cover that can be removed for regular cleaning.

      • Mats stored on top of napping cots were in visible disrepair (rips and tears). Mats were removed during inspection.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • Operator indicated that toys are on a biweekly cleaning schedule. Toys must be cleaned on a weekly basis using the 3 sink/bucket method (wash with soap and water, rinse, and disinfect in 100ppm bleach).

      • Hard to reach areas such as shelving, flooring, and closets are not visibly clean. Ensure that hard to reach areas are thoroughly cleaned and free from clutter.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • Toddler room 2: Washroom faucet was leaking during inspection. Fix faucet so that leak is no longer occuring.
