      • 1. Light covers absent on the lights in the back prep area. Please obtain2. Paint on the shelves in the FOH and BOH are peeling and no longer smooth, cleanable, and impervious to moisture. Please repaint/refinish shelves3. Caulking along sink in the back prep area is mouldy and in disrepair. Operator indicated the caulking is in the process of being replaced4, A baseboard tile is missing in the transition area between the FOH and BOH. Please replace5. A broken tile was observed on the wall at the sink area. Operator indicated this was in the process of being replaced

      • Dirty cleaning cloths were stored on the food counters.-Ensure that cleaning cloths are stored in sanitizer in between uses and regularly changed.

      • Staff observed not wearing hair nets.-Ensure that the staff is wearing hair nets or some kind of hair restraints.

      • Bulk onions were observed to be stored in a milk crate on the ground. Please ensure that all food products are stored at least 6 inches off the ground

      • Paper towels not located in paper towel dispenser. Operator indicated that they are in the process of repairing the dispenser

      • Gap exists under the back door. Please replace weatherstripping to prevent potential pest entry
