- At the time of the inspection, the facility did not have a high-level disinfectant for use during the reprocessing of their semi-critical reusable equipment (i.e. nail clippers, cuticle pushers, ect.).The facility had accidentally purchased preempt RTU, a intermediate level disinfectant during their last product order and were using that instead of the previously used preempt CS20.Ensure that an appropriate high-level disinfectant is available onsite for use on semi-critical reusable equipment, and that the disinfectant test strips available onsite match the purchased product.

- - Linens observed being hung in the mechanical remove near the furnace and hot water tank.Please ensure linens are properly laundered and storage in an appropriate location to minimize the risk of contamination.

- - The test strips for monitoring the effective minimum concentration for the PRE-empt CS 20 high-level disinfectant is expired. Expiry date observed is April 30, 2024. Please ensure test strips for monitoring high level disinfectant concentration are not used beyond their expiry date to ensure proper functioning.

- - The PRE-empt CS 20 high level disinfectant used to disinfectant semi-critical instruments is expired. Date of expiry observed is September 5, 2024. The facility has no other high-level disinfectants to replace the expired high-level disinfectant at the time of inspection.Please ensure disinfectants are not used past their expiry dates to ensure adequate effectiveness when performing disinfection of equipment and to adhere to manufacturing instructions for use.

- - The facility is not being kept clean, sanitary, and organized. There is evidence of inconsistent cleaning of general surfaces and workstations, as well as widespread disorganization of personal service tools, and equipment throughout the facility.Please conduct a thorough cleaning of the facility and ensure an organized arrangement of personal services tools and equipment to minimize contamination.
- 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
- - In the reprocessing area of the personal service facility, incompatible activities were noted. These include kitchen utensils in the reprocessing sinks, two installed gas-powered hot plates, various kitchen utensils on the counters, and an assortment of dry goods.Please ensure food related equipment and foods are removed from the reprocessing area to minimize the risk of contamination to personal services equipment.

- Expired topical antibacterial cream (poly sporin) observed in wax room.Operator discarded during inspection and was reminded to verify expiration dates of products in the facility.
- 20. Is reusable equipment disinfected properly?
- 1) High-Level Disinfectant (CS20) in use for manicure/pedicure tools.High-Level Disinfectant log not maintained and no method of testing this product was available. PHI provided operator with the High-Level Disinfectant log and discussed the significance of maintaining this log. Operator appeared to be agreeable to maintaining this log.Corrective Action:Please maintain the High-Level Disinfection log, including recording test strip results.2) Corrected during inspection: Chlorine bleach previously used in pedicure foot basins. Chlorine bleach does not have a DIN and therefore, cannot be used for disinfection in a Personal Services Facility. Operator confirmed that they understood and have switched to a quat-based disinfectant product with a DIN.
