      • 1. Infant room 1: The main sink (Adult) was not draining well (very slow drain). 2. Toddler 4: - The hand sink in the room had low water pressureEnsure all plumbing is in proper working condition.

      • Cloth dividers were used for privacy at the following washroom areas:Preschool 1(Blue) and Preschool 2 washrooms.If dividers are to be used, ensure that they are made of a material that can be cleaned and disinfected.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Sensory tables were noted to be open including water and sand tables.Discussed water tables with staff who indicated water tables are changed daily - ensure that the water is changed at least two times per day or between groups. The table and toys should be drained, cleaned and disinfected prior to use again.Children should wash their hands before and after using group sensory play.

      • 1. Preschool 3: The main sink (Adult) was not draining well (very slow drain). Staff and Director aware - maintenance requested.2. Toddler 4: Adult diapering hand sink - the hot water tap was not working. Staff were washing hands at child size hand sink with children. Ensure all plumbing is in proper working condition.

      • Toilet paper was available but not in proper dispensers in the following rooms:Preschool 1/Toddler 3 (red)Toddler 1 and Toddler 2Please ensure toilet paper is kept in the dispenser to prevent cross contamination.

      • Sponges were noted throughout the program rooms. If using sponges, they should be used once or put in approved disinfectant between uses.

      • Ceiling tile was water-damaged near the sprinkler head in Preschool 2 room. Please replace.

      • 1. Chlorine bleach disinfectant spray bottles were not labelled in Toddler 2, Preschool 3 and OSC rooms. It was addressed during the inspection.2. Chlorine concentration was over 10000 ppm in spray bottle used in OSC room. It was removed during the inspection.3. Improper disinfectant (vinegar, no DIN) was used for toys disinfection in Preschool 2 room. It was discussed with the educator.4. Baking soda was kept in the accessible sink cabinet in Preschool 2 room (cabinet was unlocked). It was addressed during the inspection.

      • 1. Plastic toys storage bin was damaged and had sharp edges in Toddler 1 room. It was removed during the inspection.2. Laundry room was accessible (door was propped open) during the inspection. Ensure that laundry room remained locked and inaccessible to children, or remove all chemicals stored in the laundry room from children's reach.
    • 06. Are proper protocols being followed in regards to client care?
      • Improper diapering procedure was observed in Toddler 2 room: the educator did not wash her hands or child's hands after diaper changing and only changed gloves. In addition, the diapering sink was inaccessible and used for toys storage.

      • Ceiling tile was water-damaged near the sprinkler head in Preschool 2 room. Please replace.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • Some antiseptic pads observed in the first aid kit expired in 2017 (new ones were also available). It was addressed during the inspection.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • Cots were not labelled in the Infant 1 room. It was addressed during the inspection.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • Some children's bottles were not labelled in Infant 1 and Toddler 2 rooms. It was addressed during the inspection.

      • Preschool 2 Room- Used wet cloth observed stored on top of bleach bottle. Please ensure re-usable cloths are placed in a bleach sanitizer after use or use disposable paper towels

      • Diaper pads damaged in Toddler Yellow diaper-change area. Please replace damaged diaper pads

    • 08. Is this facility properly constructed (Non-Critical)?
      • Water stains were observed on the ceiling in the Toddler 1 Green and Toddler 3 Red rooms, and laundry/storage room. Investigate and remove the souce of water-infiltration, and replace affected tiles.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • Antiseptic wipes observed in the small first aid kits expired in October 2017.
    • 16. Does the social care facility have adequate lighting?
      • 1. No light was available in the Toddler3 Red/Preschool 1 Blue diaper-changing area and Toddler 1 Yellow/Toddler 2 Purple diaper-changing area, and only spotlight with low lighting level (could only be turned on by pushing on the light and turning off by itself) was provided in the Preschhol 3 Yellow washroom located far from the room entrance.2. One light fixture was not in operation in Toddler 1 Green room.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. Diaper pads were damaged in the Infant 1 Red room and Toddler 1 Yellow/Toddler 2 Purple diaper-changing area. Please replace damaged diaper pads.2. Shelving unit was damaged and unwashable (unfinished surface) in the OSC room. Please repair, replace or removed affected shelving unit.

      • Hot water temperature was 51.6C in the Infant 2 Green washroom (child-size sink), 50.4C in the Toddler1/Toddler 2 washroom, 50.4C in the Preschool 3 Yellow washroom sink near the entrance to the room. Recommended maximum is 49C.
    • 30. Are workers washing hands when appropriate and/or in a proper manner?
      • Some hand sanitizers had expiry date April 2022.
