- Observation (CORRECTED DURING INSPECTION): Front doors were observed being wide open and two live birds flew into the facility during this time. Pests may enter into the facility and contaminate food and food contact surfaces.

- Observation: Wood cabinets in the front serving area behind the cash register are chipped, cracked and rough. The surface is not smooth, sealed or easily cleanable. This will attract pests. This was mentioned on two previous inspection reports and not corrected.

- Observation (CORRECTED DURING INSPECTION): Found sandwich bagettes, pizza breads, and pepperoni sticks wrapped in bread being stored at room temperature (21C). Staff indicated that the items are kept at room temperature until the end of the day and then placed in the fridge for overnight storage, and discarded at the end of the next day if not sold. This is unsafe as the food items contain protein and will allow bacteria to grow in, and could make people sick.
- Corrective Action(s): Immediately develop a formal process for holding potentially hazardous foods at room temperature, and train all staff on this process. The process shall include: holding food for ONLY 2 hours and then discarding it; and a method with which staff know when 2 hours has elapased (timers, writing time on food products, recording sheet).
