- Pest Control Checklist needs to be filled out monthly and keep it in a binder as your records.
- 19. Does the facility have a valid permit and does the facility operate in compliance with the conditions of that permit, or is the facility awaiting approval prior to operation?
- Operator posted expired Food Handling Permit (expired September 30, 2023) on the wall at the cashier area. Operator showed the permit fee paid on August 15, 2023. A copy of Pest Control check list will also be emailed to operator to record monthly pest activities.
