      • Sauce containers were observed on the floor.-Ensure that the food containers are stored at least 6 inches above the floor.

      • Multiple open containers (chicken wing, honey mustard, barbeque, chipotle sauces) were observed stored at room temperature, despite label indicating that refrigeration is required after opening. - Operator discarded the products.- Ensure to store all food items as per manufacturer's instructions.

      • Cheese and vegetables in the inserts of the prep‑line cooler were measured at 7°C. Paneer was measured at 11°C. It was also observed that the vegetables and cheese were filled above the insert fill line.-Ensure that the perishable food is stored at 4 degrees C or less under refrigeration.-Ensure that the food in the inserts does not exceed the fill line to allow proper cooling.

Operator placed cover over the food products.- Cover all food products and store in a sanitary manner to prevent from any contamination.2. Three opened metal cans opened with food were stored in the walk-in cooler. Operator transferred food items into food-grade containers.- Operator is to ensure, once canned food item is opened, food should then be transferred to a food-grade container.

      • Sewer gas odor detected near the 3-comp sink in the back kitchen. As per the operator grease trap was last cleaned in January 2025 by themselves. Grease trap cleaning equipment was improperly stored besides the grease trap. - Please repair the grease trap if required and eliminate the odor.- Operator is to ensure to store dirty cleaning equipment away from food areas.

      • There were no receipts for the tray of samosas in the walk-in cooler. Owner stated they were from an approved source however confirmation could not be provided. PHI contacted owner of restaurant where samosas were made and confirmed they were purchased there - receipts must be provided for each order.

      • A surface sanitizer was not prepared and readily available for use during operation. Please provide a surface sanitizer at each open food prep area.

      • Open foods are being handled in the enclosed dry storage room without direct access to a hand washing sink. Do not handle open foods in the dry storage room - use storage room for packaged foods only.

      • Processed foods were stored in rubber containers that were not foodgrade. Please use clean, foodgrade containers.

      • A boxed pizza from the day before was left in the display cabinet with the power turned off (destroyed). Store hot perishable foods at 60C or higher.

      • 1) The paper towel dispenser at the prep line hand sink was empty. Please restock.2) The paper towel dispenser at the 3 comp sink was empty. Please restock.3) The paper towel dispenser for the hand sink in the staff bathroom was empty. Please restock.

      • There was no cold water provided at the pizza prep line hand sink. The cold water tap fixture was turned off as it needed to be repaired. Please repair and restore cold water.

      • No pest control records available upon request.- Pest control is monitored by the operator on a routine basis and self monitoring checklist will be sent to the operator to be completed at least once a month and kept onsite for recordkeeping.

Store food packaging materials at least 6 inches off the floor and in a sanitary manner.3) The canopy filters above the rear cook line were greasy and dusty. Please clean.4) The prep counter in the back prep area with deep fryers was improperly leveled with pieces of cardboard under the counter legs. Remove cardboard and properly adjust lower shelf height.5) The lid of the chest freezer in the rear prep area was taped together. The front of the freezer also had several rust spots.
