      • Used cleaning rags were stored on the counter. - Ensure to replace cleaning rags after each use and do not store rags on the counters.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • The ceiling vents had dust accumulated. - Clean the vents.

    • 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
      • QUAT sanitizer solution in the spray bottles and from the dispenser at 3-comp sink was measured at 400 ppm.- Operator is to ensure sanitizer dispenser concentration is adjusted to 200 ppm.- Use test strips regularly to ensure appropriate sanitizer concentration.
    • 06. Are appropriate food handling practices and plans in place to ensure that food is handled in a manner that makes it safe to eat?
      • One open canned-food container of black olives was observed stored in the walk-in cooler at the time of inspection.- Food item transferred to food-grade container. - Operator is to ensure once canned food item is opened, food should then be transferred to a food-grade container.
    • 07. Do food handlers ensure that food is protected from contamination (unless noted elsewhere)(Critical)?
      • Paper towel rolls used to dry hands were placed directly above the pizza dough preparation table and dough machine posing risk of contamination to the food preparation area.- Operator immediately removed the paper towels.
    • 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
      • QUAT test strips available at the facility were discolored and expired.- Acquire valid test strips.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • Soap dispenser at the kitchen hand sink was empty and no soap was available for handwashing.- Operator refilled the soap dispenser during inspection.- Ensure hand wash sinks are fully stocked at all times with soap, paper towels, hot and cold-water supply
    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • 1. Visible gaps were noted in the back door - light can be seen coming through gaps. - Repair back door or replace weather stripping to ensure that all exterior doors are properly closed to prevent pest entry.2. No previous pest control records were provided at the time of the inspection.- Ensure pest control records are being maintained onsite at all times. Operator will be emailed AHS pest control checklist.
    • 19. Does the facility have a valid permit and does the facility operate in compliance with the conditions of that permit, or is the facility awaiting approval prior to operation?
      • Food handling permit was outdated and not properly displayed at the facility.- Please ensure a valid food permit is displayed at the facility where it can be easily seen by public.
