- In the preschool washroom, a portable children's toilet seat was observed with several rips in it and is no longer smooth and easy to clean. Ensure equipment remains smooth and easily cleanable. The operator discarded the indicated seat during the inspection.

The blue seating area on the children's couch in the preschool room was observed with several rips and is no longer smooth and easy to clean.1. Please ensure the couch is adequately steam-cleaned, fitted with a cover which can be laundered or removed from the facility.2. Ensure the indicated couch is repaired to be smooth and easy to clean or is removed from the facility.

- In the cabinet beside the diapering area in the toddler room, children's personal items such as lotions were observed to be stored in contact with one another which makes them prone to contamination. Ensure personal items are stored separately to help prevent potential contamination.

- A fabric towel was observed on the dedicated diapering surface beside the diapering pad in the toddler washroom. Ensure the surface remains resistant to moisture and easy to clean to help facilitate effective cleaning and disinfection. The operator removed the indicated towel during inspection.

- 1. A white shelf (4 feet high) observed in Preschool room and brown shelf (5 feet 11 inches high) observed in the OSC room were not secure/stable. Secure the shelved to the walls.2. Shaving cream with the warning label was observed in the Preschool room and stored in location accessible to children. It was removed during the inspection.

- Cleaning was required in multiple areas: inside sink cabinet in the Preschool room, walls and surfaces throughout. Cleaning checklist has not been maintained since September 2024. Ensure that deep cleaning of all surfaces and walls is completed, and regular cleaning is conducted daily and recorded in the cleaning checklist.

- Some sleeping mats were damaged in the Baby room. Please repair or replace.

- Children clothes were overlapping on the hooks. Bags were provided for storage of children clothes but not used. Ensure that the clothes are stored inside the bags, or have the storage hooks installed at least 12 inches apart.

- Toilet paper rolls were stored on toilet tanks, and no toilet paper dispensers were provided in the Toddler washroom. Please ensure that proper toilet paper dispensers are provided and used.

- 1. Chlorine bleach container was stored in washroom sink cabinet accessible to children in the OSC room. It was removed during the inspection. Ensure that all chemicals are kept away from children's reach.2. Chlorine bleach spray bottle was unlabeled in the Preschool room. It was addressed during the inspection. Ensure that all chemicals are lablelled.

- Push-in pins were observed in the children's areas in the Baby and Toddler rooms. Please remove.

- Oxivir Tb disinfectant expired on June 29, 2023. Please discard expired disinfecting products.

- Ceiling tiles were water-damaged in the Baby room near the back entrance door. Please investigate and remove the source of water infiltration, and replace the affected ceiling tiles.

- Ensure adequate supply of items in the first aid kits: some were missing band aids and/or antiseptic wipes.

- A 23"-high slide was observed in the Toddler room, and no protective surfacing was available. Provide appropriate protective surfacing (e.g. two layers of foam mats with 6-feet protective zone), or remove the slide.

- Storage of sleeping mats in the Preschool room was inappropriate: the adjacent mats with sheets on were not separated during the storage (bags were not covering the mats completely).
