    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures were not established for skin cleansing prior to services such as pedicures, and waxing services, and for accidental skin cuts and injuries.AHS templates were provided during the inspection.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Single use disposable nail files were stored in a used state in the dry heat sterilizer. Ensure all single use, porous, uncleanable items are discarded after each client service.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Combs were properly cleaned prior to immersion in Barbicide low level disinfectant.*** Reusable manicure and pedicure equipment were rusted. Tweezers, scissors by the waxing area were visibly contaminated with wax. Lash extension tweezers stored in the makeup container was contaminated with lash adhesive.
    • 20. Is reusable equipment disinfected properly?
      • Barbicide low level disinfectant and Clippercide intermediate level disinfectants were available or use on reusable hairstyling equipment.*** Barbicide solutions were not changed daily, as require by the manufacturer's instructions for use (MIFU)*** There was a bottle of Gluterate high level disinfectant that was expired since September 2012. A dry heat sterilizer was used for reprocessing, which is not an acceptable method of disinfection or sterilization.Corrective Actions:- Acquire an appropriate high-level disinfectant for semi-critical equipment such as tweezers, nippers, clippers, and cuticle pushers. Following the MIFU for reuse periods, and contact times.- If reusable the high level disinfectant, purchase and use test strips, and document test results in a log sheet.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • Reprocessing area was cluttered, and did not have an appropriate immersion tray for disinfection, or space for contaminated reusable equipment, and cleaned equipment.Corrective Actions:- Remove clutter, the dry heat sterilizer to allow sufficient space for reprocessing equipment.- Obtain a container to so that high level disinfectant can be used to disinfect reusable equipment.
