- Observation(s): A review of weekly menus reveals that not every meal contains at least three food groups, and not every snack contains at least two food groups, as described in Canada's Food Guide. When each meal does not contain at least three food groups, and each snack does not contain at least two food groups, this increases risk to persons in care's overall health and nutritional status. Submit by July 3, 2026, evidence that within all weekly menus, each meal contains at least three food groups, and each snack contains at least two food groups, as described in Canada's Food Guide.

- Observation(s): A review of rooms and common areas reveals an electrical cover plate missing on an electrical outlet in an upstairs bathroom. Leaving electrical outlets exposed and without cover plates increases risk of electric shock and injury to persons in care. Submit by July 3, 2026, evidence that all electrical outlets are maintained in a safe and clean condition.
