    • R2.2 - Are written policies and procedures in place to guide staff in fall prevention?
      • Observation(s): A review of restraint documentation for persons in care (PIC) shows that an agreement to the use of restraints given in writing by the medical practitioner or nurse practitioner is missing. Without agreements to the use of restraints given in writing there is potential for restraints to be used in error. Submit by April 9, 2026, evidence that for all PICs who use restraints, restraint documentation is completed.
      • R2.2A - A restraint may be applied in an emergency or when there is written agreement to the use of a restraint by both the person in care or their representatives, medical practitioner or nurse practitioner; 74( 1 )(a)(b)(i)(ii)
    • R7.2 - Is the environment maintained to prevent falls?
      • Observation(s): On a review of equipment and furnishings for use by PICs, it is observed that the laminate finish on the kitchen cabinets is peeling in several areas. There is also paint wearing on surfaces surrounding the kitchen cabinets. When furniture and equipment for use by PICs is not maintained in a good state of repair it increases risk to PICs as surface materials may contaminate food during food preparation, thereby, affecting the health and safety of PICs. Submit by April 9, 2026, evidence that all rooms and common areas will be maintained in a good state of repair.
      • R7.2N - Ensure furniture and equipment for use by persons in care are maintained in a good state of repair; 21(c)
    • R10.4 - Are restraint and fall prevention plans appropriate?
      • Observation(s): A review of restraint monitoring documentation for PICs shows that monitoring documentation is not initiated for one type of restraint utilized. The use of a restraint without assessment and monitoring puts PIC's safety and physical and emotional dignity at risk. Submit by April 9, 2026, evidence that for all PICs requiring a restraint, restraint monitoring documentation is recorded throughout the use of the restraint.
      • R10.4C - Monitor the safety and physical and emotional dignity of the person in care throughout the use of the restraint and assessed after the use of the restraint, 73( 1 )(c)
    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): On a review of rooms and common areas, it is observed that flooring is missing surrounding the floor drain in the laundry room. PICs use this area as walk-through access to each side of the facility. When rooms and common areas are not maintained in a good state of repair, this increases risk to PICs as this creates a potential tripping hazard. Submit by April 9, 2026, evidence that all rooms and common areas will be maintained in a good state of repair.

    • R9.2 - Does the supervising pharmacist consult with employees respecting medication interactions and other problems related to medication?
      • Observation(s): The most recent medication safety and advisory minutes that were available at the facility were dated September 2021. There was not a plan in place for another meeting at the time of inspection.
      • R9.2A - Appoint a supervising pharmacist to consult with employees respecting medication interactions and other problems related to medication; 68( 2 )(c)
    • R2.1 - Are written policies and procedures in place to guide employees in all matters relating to the care and supervision of persons in care?
      • Observation(s): The emergency supplies were inspected and there were seven food items where the best before date was past the inspection date.
      • R2.1G - Have a plan that sets out how persons in care will continue to be cared for in the event of an emergency; 51( 1 )(b)
    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): During inspection of physical facility, bedding items such as blankets and pillows were noted to be stored directly on the floor within the hallways storage closets. In one of the bathrooms, there were two mens electric shavers were stored in the same container on the counter each with different names of persons in care, along with a razor handle with no name.
      • R7.1J - Maintain all rooms and common areas in a safe and clean condition; 22( 1 )(c)
    • R9.1 - Are medications stored, handled, and administered appropriately?
      • Observation(s): A review of medications kept within the medication cupboard, there were three containers of blood glucose strips and one tube of a medicated cream with an expiry date that was past due.
      • R9.1I - Medications must be returned to the dispensing pharmacy if the person in care is no longer taking the medication or if the medication has expired; 72(a)(b)
    • R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
      • Observation(s): Review of the recreational care plans for persons in care showed evidence of long term recreational goals of care, but no evidence of day to day goals of care for each person in care's recreation. Please ensure documentation is completed for both day to day goals and long term goals that have been met. This was noted on previous routine inspection in 2021 as well.

    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): Facility currently does not have a consistent system in place to audit water temperature to ensure it does not exceed 49 degrees Celsius in areas accessible to PIC's. Shed in the back yard of the facility was open at the time of this inspection. ensure yard maintenance and potentially hazardous materials are kept inaccessible to PIC's. This item resolved at the time of inspection.
      • R7.1C - Ensure water accessible to a person in care does not exceed 49 degrees Celsius; 17
      • R7.1AK- Provide appropriately furnished and equipped areas for secure, safe and adequate storage of cleaning agents, chemical products and other hazardous materials; 35( 1 )(c)
    • R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
      • Observation(s): Ensure medication care plans include PRN protocols when necessary, specifically for behaviours. Recreation care plans discussed/reviewed during this inspection. Some recreation information specific to PIC's was discussed, but was not included in recreation care plan information. Ensure recreation care plans give specific details for each PIC including recreation needs within the home and away from the facility, and are updated as needed.
      • R10.3B - Care plans must include a plan to address medication, including self-medication; 81( 3 )(a)(i)

    • R10.4 - Are restraint and fall prevention plans appropriate?
      • Observation(s): One restraint currently in use, does not have a defined care plan in place, the plan should also include sign off by PIC's representative and GP.
      • R10.4G - Document in the care plan the use of the restraint, its type and the duration for which it is used ; 73( 2 )c
    • R4.1 - Are person in care records current, complete and kept confidential?
      • Observation(s): The documentation of monthly weights was missing for several months in the charting reviewed for one PIC.
      • R4.1R - Weigh each person in care monthly and record their weight in their nutritional plan (Does not apply to Hospice); 83( 4 )(a)(c)
    • R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
      • Observation(s): Facility has two hot water tanks that supply water to areas which PIC's can access, hot water temperature audits are frequently completed for only one of the tanks.
