      • *Unwrapped tea bags were observed stored in a glass jar without adequate protection from contamination. Revise the storage practice. Unwrapped tea bags must be kept in a clean container with a secure lid to prevent contamination. Ensure utensils are provided to allow residents to handle tea bags without direct hand contact. Change practice. Tea bags were discarded.

      • *Staff personal items (e.g. lunch bag, keys and cellphone) were found in the kitchen next to food preparation areas. Relocate and ensure all personal items are stored away from the food preparation areas to prevent cross contamination. Personal items were relocated to the staff room.
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • *Stand alone industrial slicer was dirty, there was debris build up on the blade and underside from the last time it was used. Immediately clean and sanitize mixer.*Two plastic cups found in the clean storage area were dirty. Both had some food debris. Re-clean and sanitize and ensure only clean cups are placed back into storage. Cups were moved to dishwashing.

      • *Dust accumulation was noted on the ceiling vent above the hand sink and AC units on the ceiling above the food preparation area across from the dishwashing area. Clean the equipment and ensure more routine cleaning is completed in addition to deep cleaning by a professional company.

      • *The flooring in the walk-in cooler was worn down. No longer smooth or easy to clean, rust color noted. Ensure floor is smooth and easy to clean.

    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • *Multiple light fixtures were burnt out in the walk in cooler and walk in freezer. Please repair/replace light fixtures so adequate lighting is available to facilitate cleaning.*Three ceiling tiles outside the walk in cooler and walk in freezer were acoustic ceiling tiles. Some tiles also had moisture stains. Acoustic ceiling tiles were not smooth or easy to clean. Please replace with smooth and easy to clean ceiling tiles. Ensure there is no leak above the tiles, repair as needed.

      • *Debris was found in equipment/utensil storage drawer by the pass-through window. Please clean out drawer and ensure sanitation is maintained. Corrected on site: staff placed equipment/utensil into dishwashing and cleaned out drawer. *Single use utensils stored by the pass-through window were improperly stored. Discussed handles should be in one direction to prevent contamination of the food end. With clean hands, please invert. Corrected on site: staff inverted utensils.

      • *Unfinished wood shelves were installed in the dry storage room. Finish wood shelves and ensure they are smooth and easy to clean.

      • *Test strips were not available to confirm quat sanitizer and bleach sanitizer concentrations. Please get test strips for staff to use to confirm adequate sanitizer concentration.

      • *Observed used cleaning cloth on counter in server area. Ensure that cleaning cloths are stored directly in sanitizer when not in use. Cloth removed from food contact area during inspection.

      • *Food boxes were observed to be stored on the floor in the walk-in freezer. Ensure all food boxes are stored above floor. Remove food boxes immediately.

      • *Paper towel was missing at the hand washing station. This was corrected during inspection. Ensure supplies are always available for hand washing/.

      • *Mice droppings were observed in the dry storage area. Areas were showed to the operator. Ensure areas are adequately cleaned (as discussed onsite) and pest monitoring/control is done immediately.
