      • Noted that the main walk-in cooler was out of order as it was smashed by a vehicle from outside (occurred beginning of October 2023). Foods (dairy products and fresh produce) were observed to be stored in a portable rental cooler placed outside of the building. Foods in the rental cooler were measured at satisfactory temperature (<4C). Nov 8: Manager/operator advised the walk-in cooler repair in progress; New walk-in cooler has been ordered but it gets delayed due to supply chain issues.

      • * Many flies were observed flying around in the kitchen and on the tables in the dining area.* Advised that pest exterminator is working on the issue. Pest control record (dated July 17) from a pest exterminator was reviewed. The report indicated evidence of flies. * Advised that cleaning of floor drains was conducted on July 18.
