      • Low- level disinfectant used for immersing combs was at inadequate concentration. Staff indicated unclean combs are stored in the solution to be cleaned at the end of day.Ensure combs are cleaned and rinsed before immersing in the barbicide solution at adequate concentration.

High level disinfection logs were not observed and maintained. Please maintain high level disinfection logs.
