      • Holes were noted on the floor located in the main prep area as well as underneath the equipment. Please repair the floor.

      • 1. It was noted that the quat auto dispenser was dispensing quat solution at 50 ppm. Please calibrate the quat dispenser. In the interim, please manually mix the quat solution and ensure it is maintained at 200 ppm. 2. Cleaning cloths were being used as oven mitts and noted to be stored on prep counters. Please acquire oven mitts. Please ensure all cleaning cloths are stored in a quat solution all times.

      • Food handlers were not washing hands between tasks. Importance of hand washing was discussed with the operator during the inspection. Hands must be washed between tasks.

      • Scoops were noted inside bulk food containers. These were moved during the inspection. Please do not store scoops inside bulk containers.

      • 1. Holes were noted on the floor located in the main prep area as well as underneath the equipment. Please repair the floor. 2. Paint was noted be peeling by the wall located by the single prep sink. Please refinish the wall surface.

      • Food debris noted on customer utensils storage containers. These were washed during the inspection.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • General cleaning is required in the facility. Accumulation of grease was noted underneath the equipment and inside deep friers and mechanical dishwasher, ventilation hood of the dishwasher and inside drawers. Please clean the facility and ensure it is maintained in a clean and sanitary manner at all times.

      • Bare plywood wall and shelves were noted in the dry storage area. Please ensure all surfaces are made of smooth, easy to clean and moisture resistant state.

      • Cleaning cloths were noted on prep counters. All cleaning cloths were moved into the sanitizing solution during the inspection. Please do not store cleaning cloth on prep counters.

      • Food handlers were observed to be wiping hands using cleaning cloths and were unaware of proper hand washing procedures. Proper hand washing procedures were discussed with food handlers during the inspection. Please ensure to wash hands properly.

      • A weather strip on the back door is not adequate. Please install an appropriate weather strip.
