- 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
- Paper towels were not supplied at the hand sink in the washroom. Install a paper towel holder on the wall and supply paper towel at the hand sink.

- 1-No expiry date on Preempt CS20 and AHP7 test strips. Both had passed their expiry date according to the HLD test log. There were no expiry dates on the containers. The part of the label containing expiry date was removed from the containers.

- No HLD testing log was maintained since April 2022. Operator will dump HLD everyday as facial clients are not many. No re-use of HLD in future. No HLD testing is required if every day new solution is used for disinfection of semi critical tools.

- PreEmpt RTU bottle was noted being expired. Obtain at minimum low-level disinfectant with valid expiry date for environmental surface cleaning.

- Hand washing was being done in the reprocessing sink. Reprocessing was being done without decontamination of the sink.Follow the facility procedure for decontamination of dual purpose sink before any reprocessing of semi-critical tools.
