    • 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
      • QUAT test strips onsite were expired in January 2023. Obtain new valid test strips. - December 5, 2024: no test strips available onsite staff was unsure of the type of test strips required. Discussed sanitizers and their required test strips with staff member during the inspection.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • Countertop at the 3-compartment sink was in disrepair and requires repair/replacement. Ensure the counter is smooth, non-porous, and easily cleanable. Pieces of the finished surface were missing and the venire finished surface was peeling away from the unfinished base gaps noted throughout countertop area.Microwave shelf was in disrepair and requires repair/refinishing to be smooth, non-porous, and easily cleanable.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • The following items/areas were visibly dirty and require cleaning:- microwave- lower cabinet drawers and the inserts and utensils/tools insideCleaning checklist/schedule available but not completed since June. Ensure the checklist is completed regularly and dated (include the year).

      • Hand sink in the food prep area did not have hand soap and staff indicated that none was available onsite to restock.Ensure the hand soap is restocked prior to any food preparation in this area. Strongly recommend obtaining a backup supply.

      • Various frozen food products were stored on the floor of the walk-in freezer. Ensure all food products are stored at least 6 inches off the floor and protected from contamination.

      • QUAT test strips onsite were expired in January 2023. Obtain new valid test strips.

      • Pest monitoring inspection records were not completed. Ensure regular pest monitoring inspections are completed and recorded and those records are kept onsite available for review.
