    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. The facility required additional cleaning and organized. Staff personal items were noted to be stored with daycare items.Please clean and organize the facility thoroughly and ensure it is maintained in a clean and sanitary state at all times.2. Debris was noted on OSC tables and chairs. It was noted that facility was not disinfecting surfaces between tasks. Please use a bleach solution to disinfect all surfaces.

      • Toys were not being washed properly. Discussed proper toy washing procedures. Please either use mechanical dishwasher after food activities area ceased for the day or use two compartment sinks located in the kitchen. Wash with soap and water and rinse with clean water. Disinfect using bleach solution at 100 ppm for two minutes.

      • A gap was noted between the baseboard and floor in the baby room besides children hand washing sink. A transition strip was noted to be missing in the infant room. Please seal the gap.

      • Diaper creams were noted to be stored within client's access. One client was noted to be grabbing diaper cream and noted to be eating it.All diaper creams were moved during the inspection. The operator was advised not to store chemicals with in client's reach.
    • 06. Are proper protocols being followed in regards to client care?
      • 1. It was noted that the facility was not following proper diapering procedures. Proper diapering procedures were discussed with the operator during the inspection. Please ensure to train staff members and follow proper diapering procedures.2. Cracks were noted in the diapering pad located in the washroom. The operator discarded the diapering pad during the inspection. Please ensure that all diapering surfaces are intact at all times.

      • A gap was noted between the baseboard and floor in the baby room besides children hand washing sink. Please seal the gap.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • It was noted that the facility was not washing toys properly. Proper toy washing procedures were discussed with the operator during the inspection. The operator was advised to create a toy washing procedure and schedule and ensure to follow it.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • Clients' personal items were in physical contact with other client's personal items. Please be advised to separate client's personal items and ensure they are not in physical contact with other client's personal items. The client's items were properly stored during the inspection.
    • 30. Are workers washing hands when appropriate and/or in a proper manner?
      • Staff members were not washing hands between tasks. Importance of handwashing was discussed with the operator during the inspection. Please wash hands between tasks.

      • 1. Flooring was noted to be damaged throughout the facility. Please repair the damaged flooring. This was not observed as the inspection was conducted during nap time. 2. Ceiling tiles were noted to be damaged throughout the facility. Ceiling tiles were bulging out at multiple locations. Please investigate the source of this concern and repair all damage and replace all damaged ceiling tiles.

      • A diapering station was not observed in the washroom area. Please acquire a diapering station for washroom area.

      • 1. Flooring was noted to be damaged throughout the facility. Please repair the damaged flooring. 2. Holes and cracks were noted in the walls. Please seal all wall damage and ensure all walls are maintained in a smooth, easy-to-clean and moisture resistant state. 3. Ceiling tiles were noted to be damaged throughout the facility. Ceiling tiles were bulging out at multiple locations. Please investigate the source of this concern and repair all damage and replace all damaged ceiling tiles.4. Holes were noted in the walls inside shelving cabinets of all hand washing sinks. Please seal all these holes. 5. Exposed insulation was noted inside bathroom sink cabinet. Please cover the insulation.

      • Bite marks were noted on soft foam toys during the inspection. Please discard all foam toys with bite marks.

      • 1. A sink of the washroom located by the backdoor was noted to be very small. Please replace this sink.

      • General cleaning and organizing is required throughout the facility. Please clean and organize the facility thoroughly and ensure it is maintained in a clean and sanitary manner at all times.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • 1. Bite marks were noted on soft foam toys during the inspection. Please discard all foam toys with bite marks. 2. The operator indicated that they toys are being cleaned using the commercial dishwasher. The commercial dishwasher was not dispensing chlorine at the time of inspection. Please repair the dishwasher and ensure it is dispensing chlorine at 100 ppm at all times. Please wash toys manually until the dishwasher is repaired. Proper toy washing procedures were discussed with the operator. Wash toys with soap and water, rinse with clean water and disinfect with bleach solution for at least two minutes. Air dry all toys.

      • Client's shoes were noted on the floor in childcare areas and were not organized. Please ensure clients personal items are stored in their cubbies at all times.
    • 28. Are washrooms / bathing facilities properly constructed, maintained and supplied (Critical)?
      • 1. A sink of the washroom located by the backdoor was noted to be very small. Please replace this sink.2. General cleaning is required in all washrooms. Please clean and organize all washrooms.

      • Staff members were not washing hands between tasks. Importance of hand washing was discussed with the operator during the inspection. Please wash hands between tasks.

      • Staff personal items were noted throughout the facility (stored among childcare facility supplies). Please designate an area to store staff personal items located away from childcare facility supply storage.

      • The diapering area bleach was noted to be at 100 ppm. The operator prepared fresh diapering area bleach and concentration was noted to be at 1000 ppm.
    • 08. Is this facility properly constructed (Non-Critical)?
      • It was noted that the facility was washing toys at the hand washing sink located in the infant area. The operator was advised to use either the dishwasher or two compartment sinks located in the kitchen area once the food handling and dishwashing is competed for the day.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • 1. Clients' personal items were in physical contact with other clients' personal items. Please be advised that clients personal items must note be in physical contact with other clients' personal items. Please either acquire large bags to store personal items or install a barrier between storage hooks.2. Gloves and paper towel rolls were noted to be stored in the washroom. These were moved during the inspection. The operator was advised not to store gloves and paper towel rolls in the washroom. 3. Staff personal items were noted to be stored with clients' items. Please designate an area to store staff personal items that is located away from clients and daycare supplies.

      • 1. The part of flooring was observed to be missing in the preschool area, and baby room. Please ensure that floor is made of smooth, easy-to-clean and durable state. Please install the missing flooring. 2. The hand washing sink cabinet wall of one of the washrooms require to be painted. Please seal the wall and ensure it is maintained in a smooth, easy-to-clean and moisture resistant state.

      • 1. A hot water was not observed by both hand washing sinks located in baby room. Please ensure that the hot water is available at both these sinks.

      • 1. The plumbing lines of two hand washing sinks located in the common washroom were leaking. Please repair or replace leaking flooring as needed. 2. The washroom requires cleaning. Please clean the washroom. 3. Bathing provisions were not observed for infants under 19 months. Please ensure that the facility has bathing provisions.
