- A cockroach was observed in a sticky trap beside the range. The pest control report from Ecopest indicated that evidence of cockroach activity was present in other areas of the school since Jan. 22, 2026.Professionally inspect, treat, and eradicate any cockroaches. Remove dead cockroaches, clean cockroach feces, and sanitize the surfaces. Monitor and adjust treatment accordingly. Send an updated professional pest control report to the undersigned PHI for assessment. A re-inspection of the school will be conducted to ensure compliance.

- The cabinet surfaces underneath the counter were observed to be water-damaged and in disrepair.Please repair, repaint, or replace the countertop cabinets to ensure the surface are smooth, easily cleanable, and impervious to moisture.

Additionally, implement a routine cleaning schedule to ensure the facility is regularly maintained.
