      • It was observed that the quats test strip being expire. - Please obtain new test strips with the color chart so you can verify the Quat-based surface sanitizer concentration (200ppm).
    • 19. Does the facility have a valid permit and does the facility operate in compliance with the conditions of that permit, or is the facility awaiting approval prior to operation?
      • The permit post expired on September 30, 2025.- Have the new permit printed and posted.

      • Use of duct tape was observed on the service table. - Remove tape from indicated areas so surfaces are smooth, nonporous and easy to clean.

      • The facility does have the Quat test strips on-site but the color chart was missing. Please obtain new test strips with the color chart so you can verify the Quat-based surface sanitizer concentration (200ppm).

    • 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
      • Reusable wiping cloths were being stored on the countertop. There was no wiping cloth bucket with sanitizer solution prepared.A wiping cloth bucket with sanitizer solution was prepared. Cloths were placed in the wiping cloth bucket. Ensure that wiping cloths are stored in food contact surface sanitizer solution between uses.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • The grease interceptor is not being adequately maintained. Last service, dated November 9, 2023, the grease trap was 100% full of FOG and solids (60% FOG, 40% solids); September 8, 2023 70% full of FOG and solids (35% FOG, 35% solids), and August 4, 2023, 80% full of FOG and solids (40% FOG, 40% solids).The amount of solids and FOG combined should not exceed 25%.
