      • The basement shared food storage area with wire mesh storage lockers had a significant cockroach infestation. German cockroaches were observed on monitoring glue boards as well as free from traps crawling along the floor.Please work with a professional pest control company to abate the pest issue,.
    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • Evidence of cockroach activity was observed in the stall. One dead German cockroach was observed in a glue board in a tin cat mouse trap. Cockroach excrement (frass) was also observed on the corner of a cupboard/ drawer under the POS system.Please work with a professional pest control company to abate the pest issue.

      • There were no designated cockroach monitoring traps present in the stall.Please add cockroach traps to the stall and work with the professional pest control company to treat the pest activity.

      • November 10, 2025:In the basement storage area:cockroach frass was observed on surfaces of the chest freezer, shredded paper/cardboard was observed within the space containing the condenser of the chest freezer, approximately 10 dead cockroaches were observed on the floor, and the upright fridge/freezer had food spillage inside.- A soiled blender was being stored on the cage above the storage unit.- A black plastic shelving unit had dead cockroaches and food debris accumulated.In the booth:- The counter top oven had food debris accumulation inside and out.- Corners and edges of floors had grease, soil, and/or food debris accumulation.- Floors under the counter with the sinks had grease, soil, and food debris accumulation. - Cooler door hinges, gaskets, and wheels also require cleaning. - Cockroach frass was present on the west wall by the front counter.November 28, 2025:The general sanitation of the stall was poor. A buildup of food debris and grime was observed along the hard-to-reach areas such as floors under and behind equipment.Please deep clean the stall.

      • Open package of frozen bananas stored by the hand sink. Food should not be stored near the hand sink as this may result in contamination from the water splashes and droplets when performing hand hygiene. Store food away from the hand sink or install a barrier. The operator moved the frozen bananas away from the hand sink at the time of the inspection.
    • 11. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Critical)?
      • The operators are not manually washing their dishes in a proper manner. No final sanitizing step is followed.Performing manual dishwashing in a two-compartment sink must adhere to the following: food equipment and utensils must be washed and rinsed in the first compartment then fully submerged in a sanitizer solution in the second compartment for at least 2 minutes. Ensure this procedure is followed to clean/sanitize food equipment and utensils in a two-compartment sink. The operator filled the second compartment with a quats sanitizer solution and measured 200ppm. A diagram for manual washing in a two-compartment sink was provided to the operator.

      • - During the inspection, two packets of Vega protein and Bubly drinks were found stored on the floor under the two-compartment sinks. Staff have since relocated these items to the shelves.*Please ensure that all food items are stored at least 15 cm above the floor to prevent contamination and maintain proper hygiene standards.

      • - During the inspection, an unlabeled sanitizer bottle was observed. Staff have since labeled the bottle.*Please ensure that all chemicals on the premises are properly labeled to prevent any potential mixing of substances.

      • - During the inspection, the paper towel dispenser was found to be empty. Staff have since restocked it with paper towels.*The hand sink in the facility must always be stocked with soap, single-use hand towels, and both hot and cold water.**Please ensure these supplies are regularly checked and maintained to uphold proper hygiene standards.

      • Concentration of quat based food contact surface sanitizer available in a spray bottle in the facility measured approximately 100 ppm.Solution was remade and concentration measured 200 ppm. Ensure that quat based food contact surface sanitizer solution is maintained at 200 ppm.

      • Quat test strips were unavailable for use.Obtain quat test strips for verifying quat sanitizer concentration.

      • The soap dispenser at the hand wash sink was empty. The paper towel dispenser at the hand wash sink was empty of paper towels.Refill the hand soap dispenser and the paper towel dispenser at the hand wash sink. Ensure that soap dispenser and paper towel dispenser are fully stocked at all times.
