- The president advised that the contractor attended the premises on Monday, November 17th as originally indicated. An inspection hole one foot in diameter was prepared to observe the extent of damage from the infiltration. The contractor advised that the water damaged building materials will be removed and crack filler will be applied both to the interior and exterior of the building. The repairs will be completed once the contractor has availability which should be prior to the anticipated shutdown dates in December.

- 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
- Significant wood root is observable along the lower edge of the east facing interior wall of the east end walk-in cooler. The lounge manager noted during the inspection that it is due to water that is infiltrating through cracks in the foundation on the other side of the walk during significant weather events.*Post-inspection discussion completed with the branch president by phone, who indicated that the issue has occurred once before and that appropriate measures were taken previously such as repairs to the foundation where water had infiltrated/is infiltrating and the removal of water damaged building materials such as the damaged plywood. The president indicated that a contractor has already been contacted and is scheduled to attend on-site on Monday, November 17th to conduct the initial assessment.

- The glasswasher is not providing a fully detectable concentration of chlorine after the unit was primed and the solution was replaced with a newer supply, indicating that there may be a mechanical issue with the unit. The office manager indicated that she would be calling the maintenance person shortly during the inspection.
