      • -Written procedures were not available for this facility. Please ensure written procedures are developed for: Preparation of Skin, Post-Service Car and Client Care.
    • 20. Is reusable equipment disinfected properly?
      • - Pre-empt CS20 was being used to disinfect equipment but equipment was not being fully immersed in the high-level disinfectant during the disinfection process. - High level disinfectants are not being tested and verified daily using proper test strips. Ensure records are kept of: test dates, whether the solution passed/failed, when the disinfectant was changed and the worker’s initials who performed the activity.Ensure the high level disinfectant is tested daily using the test strips specified by the disinfectant manufacturer.

      • Last Inspection:-The manufacturer’s instructions for the Hydrafacial machine and the Microdermabrasion machine are not kept in an accessible place. Current Inspection:- Still no instructions for the microdermabrasion unit. Ensure all booklets from manufacturer's are available.

    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • -An antiseptic product is not being applied prior to a service that will puncture the skin. Ensure an antiseptic product is used prior to PMU services.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • - Porous or uncleanable equipment is not being discarded after each use. There was an orange stick and a single use nail file soaking in disinfectant. Equipment classified as porous or uncleanable must be discarded after use in one personal service on one client.-Lancets, which are considered single use were being re-used. Ensure sharps are disposed of after each use.*Single use disposable lancets are available for purchase.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • - Used equipment was not being adequately cleaned and dried prior to disinfection. Waxing tools, the tint bowl and applicator were stored unclean. Ensure equipment is: disassembled, cleaned to remove soil, and rinsed and dried prior to disinfection or sterilization.*It is strongly recommended that a single use mixing bowl and applicator are used for lash/brow tinting.

Ensure equipment is fully immersed in solution.- High level disinfectants are not being tested and verified daily using proper test strips. Ensure records are kept of: test dates, whether the solution passed/failed, when the disinfectant was changed and the worker’s initials who performed the activity.Ensure the high level disinfectant is tested daily using the test strips specified by the disinfectant manufacturer.-Staff indicated that Barbicide is changed every 5 days. Manufacturer’s instructions indicate that it needs to be changed daily. Ensure that it is changed daily.-Alcohol was being used to disinfect wax tools; this is not an appropriate disinfectant. Ensure at minimum a low level disinfectant is used on waxing tools.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • - Sterility assurance documentation was not available for all equipment purchased as sterile (microneedling cartridges). Ensure appropriate documentation is available for equipment that is purchased as sterile.
