     • Observation(s): - The annual practice of the large scale evacuation plan was not recorded as required. *Resolved during the inspection. Fire drills were practiced monthly and the record was displayed as required. The Licensee confirmed to the Licensing Officer that no revisions to the facility policies and procedures in place had occurred since the last routine inspection.
