- The number of flies had reduced since the last inspection 4 days ago. Dead flies were observed on the floor under the electrical panels, dishwashing sinks and by the janitorial sink. Live activity was seen on windows and walls in the back of house.Remove flies and their breeding ground such as standing water, which was observed around the ice bin. The catch tray was overflowing. Containers with drink toppings were opened when not in use. Lids were placed on these food containers during the inspection.

- The food establishment had a fly infestation. Flies were observed on counters, walls, floors, windows and in the ice machine.Remove flies and their breeding ground such as standing water, which was observed around the front hand sink, under floor mats and dishwashing sinks.Opened packages of cake pops were left unattended by the drive thru. Containers with drink toppings were uncovered when not in use. Cover foods properly to avoid contamination.The use of the ice machine had been suspended prior to the inspection due to contamination of flies. It was being emptied. Clean and sanitize the ice machine before using it again.

- QUAT surface sanitizer in washcloth buckets at the front counter was tested at 100 ppm. Operator changed solution during inspection.- As discussed, ensure that sanitizer concentration is regularly checked using available test strips.

- One of the blender jugs used to make drinks at the front counter was observed to be visibly cracked and damaged at the top.- Operator was instructed to discontinue the use of damaged jug for food preparation.- Ensure all food contact utensils/equipment are maintained in a clean and sound condition.
