      • 1. Tweezers used in the waxing room is not fully immersed in the CS20. Disinfect the tweezers in the container that can fully immerse them into the CS20.

      • 1. CS20 has not been monitored for the past 4 days. CS20 level must be tested and recorded daily.2. No test strip was available. Please obtain test strip to test CS20 for reuse. PHI tested CS20 at time of visit and concentration passed. 3. Tweezers used in the waxing room is not fully immersed in the CS20. Disinfect the tweezers in the container that can fully immerse them into the CS20.4. Obtain a timer to keep track of contact time when CS20 is in use.

      • Water damaged ceiling panel in the washroom. Replace ceiling panel to ensure it's not from new water leakage.
