- Observation(s): It is acknowledged by the manager that all performance reviews have not been completed annually as required. Regular performance reviews ensure that management and the staff are aware of any skill areas that could be enhanced and is an opportunity to discuss career development. Failing to have regular performance reviews may impede the ability of a licensee to implement performance improvement plans if there are any performance issues which may pose a risk to the health safety and dignity of persons in care. Please provide a plan to ensure that performance reviews are completed as required by January 6, 2026

- R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
- Observation(s): It is noted that some person in care bedrooms have wall damage from wheelchairs or walkers, and a lift has caused damage to a wall in a washroom. The prevention and maintenance system for these issues was not clear. Maintenance of physical facility is a dignity issue for persons in care. Please provide a plan to ensure that physical facility is maintained as required by January 6, 2026.

- Observation(s): It was a determined that two persons in care residing at the facility did not meet the definition of Long Term Care in the Residential Care Regulation. The Licensing Officer discussed options with facility leadership to resolve this contravention.

- R2.1 - Are written policies and procedures in place to guide employees in all matters relating to the care and supervision of persons in care?
- Observation(s): It was confirmed that Acting Manager and Assistant Manager received little orientation regarding regulations and the Community Care and Assisted Living Act.

- R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
- Observation(s): Water temperature was measured at 52 degrees at several faucets accessible to Persons in Care across 3 floors. The temperature was adjusted and corrected during the inspection. The maintenance manager has a weekly auditing system in place which will be further enhanced.

- Observation(s): A person in care's smoking care plan lacked sufficient details including monitoring to ensure the PIC's health and safety.

- R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
- Observation(s): It was observed during the inspection that the facility has electronic devices for the purposes of transmitting or recording images of persons in care located on each floor. There was no notice prominently placed displaying that electronic surveillance was in use. During a tour of the facility it was observed that the outdoor covered patios were cluttered with seasonal furniture.

- Observation(s): It was observed during the inspection, that a fall mat had not been removed from a person in care's room as indicated on the Care plan.

- Observation(s): Concern alleged the licensee was not providing medications on time. Licensing reviewed the medication administration audit report, and the facility Director of Care confirmed not following their policy.

- R6.3 - Does the facility demonstrate appropriate outbreak prevention and control measures?
- Observation(s): Concern alleged the licensee was not providing a regular bathing routine and person in care was regularly found with food on face and in dirty clothing. Licensing reviewed the bathing documentation with no concern; however, observed the person in care in an unkempt state as alleged.

- R7.1 - Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?
- Observation(s): Concern alleged that the state of the person in care’s room is not cleaned properly, smell of urine, and dirty floor with personal possessions strewn about and the general configuration of support mechanisms i.e. falls mat and sit to stand pole restricting mobility. Licensing observed the room with food and medication on the floor, and person in care with restricted mobility/access in wheel chair. Soiled clothing was also observed on the floor under the sink in the bathroom.

- Observation(s): Concern alleged that program of activities are not meeting the needs of the person in care with respects to increasing physical fitness / activities. Licensing observed Care conference notes which referenced the need for increased physical fitness / activities, this is not reflected in the Care plan.

- Observation(s): Unlocked craft room contained potential hazards to PICs. Observed 3 unlocked medicine cabinets in PIC rooms; 2 of cabinets contained prescribed ointments.
