      • 1. Waxing pots were covered in tinfoil, which is not easily cleanable, while waxing sticks were left in waxing pots while a service was not being performed.Please ensure that wax pots are easily cleanable, and that waxing sticks are not left in the pots, double dipping does not occur, and that waxing sticks are used once on one client and then immediately discarded.

      • Ensure to test high level disinfectant daily and record. After 14 days, discard and keep testing daily.

      • Facility was not visibly clean in certain areas, clean drawers and areas throughout the facility

      • 1. Waxing pots were covered in tinfoil, which is not easily cleanable, while waxing sticks were left in waxing pots while a service was not being performed.Please ensure that wax pots are easily cleanable, and that waxing sticks are not left in the pots, double dipping does not occur, and that waxing sticks are used once on one client and then immediately discarded.2. Used nail files were observed being stored in drawers. Please ensure that disposable equipment that is porous and not easily cleanable is immediately discarded after use with each client.3. E-files appeared to be stored with used sanding bands and drill bits still attached.Please ensure to discard single-use sanding bands after each client, and clean/disinfect drill bits appropriately.
    • 20. Is reusable equipment disinfected properly?
      • Semi-critical tools were not being disinfected properly. A high-level disinfectant (PREempt CS20) was available onsite but was expired. Test strips were not present, and a testing log was also present despite the reuse of the high-level disinfectant.This must be rectified immediately, as this is a significant health hazard to clients for skin infections. Please ensure to have non-expired high-level disinfectant available by the beginning of the next working business day (July 17th, 2026).
    • 24. Are waste sharps disposed of properly?
      • While a proper waste sharps container was present, used razor blades were observed sitting on top of the sharps waste container, and a pair of tweezers was stuck in the entrance to the container. The employee present rectified the situation.Please ensure that sharps are disposed of properly, and the lid is kept closed, as this provides a physical hazard to employee who may cut themselves.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Facility was not visibly clean in certain areas, including the reprocessing sink for semi-critical tools, as well as around the bathroom sink. Towel warmers also had pooling liquid and visible dirt present in the collecting trays at the bottom. Please ensure that facility is in a visibly clean state, especially in areas where clients may have access.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Reusable dishware (wine glasses, plates, and bowls) was observed in cabinets on the client side of the salon, indicating food could be served and/or was accessible to clients. Please note that food and drinks cannot be handled and given to clients unless a valid food permit has been granted and valid dish-washing facilities are present and usable on-site.
