- During inspection 2 traps were observed with dead and alive cockroaches. Roughly 6 were observed between 2 sticky traps. Operator indicated working with pest control company.

- During inspection multiple traps were observed with dead and alive cockroaches. Roughly 9 were observed between 3 sticky traps. Ensure that weekly pest control is occurring. Treatment required to resolve issue.

- The surfaces under the handwashing sink on the south west side of the front service area is water damaged and warp. Repair surfaces so they are smooth, impervious to moisture and easy to clean.

- Golden sticky glue sticks were observed in use in the food preparation and storage area.-Please discontinue the use of sticky fly control sticks. These devices pose a contamination risk because dead or dehydrated flies can detach and fall onto food, food-contact surfaces, or utensils below. Remove all such traps from food storage and preparation areas and implement alternative pest control measures that comply with food safety standards.

- Dishwasher was empty on detergent.

- 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
- All sanitizer buckets measured at 0 ppm QUATs. It was observed that the bag the sanitizer comes out of was completely empty. A new sanitizer was connected and new sani buckets were made. Please ensure staff are using the test strips daily to confirm that sanitizer is being dispensed.

- Bacon and eggs stored at room temperature had stickers on that passed the "use by date" by 30 minutes (eggs) and 4 hours (bacon). It was observed that the sticker printer stopped working and the stickers weren't replaced. Corrected on-site.

- Handwashing sink upon entry to the kitchen needs to have a physical barrier (such as plexi glass) installed on the right of it to prevent splashing of water onto the ready to eat donuts. Alternatively, the donuts can be moved to another area or kept covered to protect from contamination.

- Multiple dead cockroaches of all life stages observed on sticky traps in the dishpit area. Please ensure all items recommended by pest control are being maintained (sanitation and structural issues).

- Plumbing maintenace required for multiple handwashing sink faucets (multiple faucets are loose, one faucet was leaking, one faucet had the cold water turned off). Manager indicated they are having the faucets/sinks all replaced soon.

- Structural issues noted throughout the facility. Please seal all holes/gaps/crevices in the facility requested by pest control to prevent harbourage of cockroaches. Part of the baseboard is missing in the back by the walk-in freezer.

- Cleaning required inside one of the coolers, next to the to go bags. Further care also required for customer to go bags as multiple were soiled/greasy.
