    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • ** Preschool Bathroom - chemicals were being stored under one of the sinks in the cupboard of the preschool bathroom. Please remove the chemicals and store them out of the reach of children or ensure there is a child proof locking mechanism on the cabinet.
    • 08. Is this facility properly constructed (Non-Critical)?
      • ** Preschool Room - The wall was in disrepair by the office in the preschool room. Repair required.** Preschool Bathroom - one ceiling tile was falling down. Please secure.** Infant room - The shelving above the sink in the baby room was covered with a foil surface sticker. the material was lifting and in disrepair. Please ensure the surface is smooth, cleanable and in good repair.** Toddler room - The wall near the windows was damaged in the toddler room.** OSC - a ceiling tile in the OSC staff bathroom was not properly secure.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • ** Preschool Room - bleach solution was measured at 0ppm in the spray bottle in the preschool room which is not adequate for disinfection of surfaces. Bleach solution must be at least 100ppm.** Preschool Bathroom - the fan cover in the bathroom was loose and could fall. Please repair.** Preschool Bathroom - exposed edges were observed on the front sides of the bathroom stall dividers. Please ensure all edges are smooth and cleanable.** Preschool bathroom- several of the staff dividers were loose. Please secure.** Preschool bathroom - The top of the cupboard in the bathroom had exposed wood and the surface was not smooth and easily cleanable. Please repair and ensure the surface is smooth and cleanable.** Preschool bathroom - a yellow wiping cloth was observed on the sink. Please ensure wiping cloths are placed in the laundry after each use to prevent bacterial growth.** Kinder room - One of the area rugs (middle of room) was lifting in the corners which could be a tripping hazard. Please repair of remove so that there is no tripping hazard.** Infant room - The table straps on the infant chairs were visibly dirty. Cleaning or replacement required.** Infant room - The wall mounted, fold down change table was coming out of the wall and was not properly secured. Please secure.** Infant room - The wood reinforcing the wall mounted fold down change table was exposed and not easily cleanable. Please finish this area to ensure the surface is easily cleanable.** Infant room - The cupboard door in the infant room was in disrepair. Repair required.** Toddler Bathroom - The change pad was in disrepair in the toddler bathroom. Replacement required.** Toddler bathroom - The cupboard in the toddler bathroom was not smooth and cleanable. Refinishing required.** Toddler bathroom - The bathroom fan in the toddler bathroom was visibly dirty and requires cleaning.** OSC Bathroom fan cover was visibly dirty

      • ** Large 4 ft stuffed animal was observed in the preschool room. Please remove from use as items this large cannot be properly washed and dried.** Toddler room - A bin for mouthed toys was not observed in the toddler room.** OSC - Large 4 ft stuffed animal was observed in the OSC room. Please remove from use as items this large cannot be properly washed and dried.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • ** Preschool bathroom - one of the toilets in the preschool bathroom was continuously running. Repair required.** Kinder room sink - the water temperature at the kinder room sink was too cold, measured to be 3.6C. Please ensure hot and cold potable water is present at all times. Note: Maximum water temperature is recommended to be no more that 49C.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • ** Preschool bathroom - toilet paper roll holders were broken, or toilet paper was not installed. Please ensure that toilet paper holders re installed, properly operational and supplied with toilet paper at all times.** OSC - Toilet paper was not observed on the dispensers in the OSC staff/children bathrooms.** OSC - Paper towel was not observed to be installed in the dispenser in the OSC staff/children bathrooms.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • ** Toddler room - No thermometer was observed in the fridge.** Toddler room - bottles in the toddler room fridge were not observed to have covers. Please ensure covers are provided or a temporary cover such as plastic bag or cellophane is used.

    • 06. Are proper protocols being followed in regards to client care?
      • The facility has the wrong chlorine test papers (i.e. used for measuring pool chemistry). These can only read up to a concentration of 10 ppm.Corrective Action:Ensure you have test papers that allow you to test the concentration of your chlorine bleach disinfectants - up to 1000 ppm.(Chlorine bleach disinfectants should be at 100 ppm for daily use, or at 1000 ppm for use during an outbreak).
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • The small, upholstered chair in the Preschool room is in disrepair such that the leather is peeling.Corrective Action:Repair this chair so that it is in good repair and easy to clean or remove this chair from the facility.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • Toilet paper dispensers are missing in the preschool washroom and the out of school care washroom. Toilet paper is available but not stored in dispensers.Corrective Action:Ensure all toilet paper is stored in a dispenser.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • The facility does not maintain an illness log with the appropriate information (including symptoms).Corrective Action:Create and maintain an illness log for both children and staff.Note: All/any cases of illness identified in children and staff must be logged and managed (e.g. isolated or excluded) by staff at the facility, in a manner consistent with the AHS Outbreak Guidelines.

      • The small, upholstered chair in the Preschool room is in disrepair such that the leather is peeling.Corrective Action:Repair this chair so that it is in good repair and easy to clean or remove this chair from the facility.

      • The hot water for the hand washing sink in the Preschool room does not work.Corrective Action:Ensure this sink provides sufficient hot running potable water at suitable pressure at all times.

      • - Cracking observed on some floor tiles in various rooms- Paint chipped on walls in various rooms
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Preschool Washroom:- No hand soap was observed at handwashing sink as Ranya communicated these sinks are not used for handwashing. Since these handwashing sinks are operable, it is highly recommended to use these as handwashing sinks. Soap was provided during inspection

      • -Preschool and Kinder Room: some lights out of order

      • Preschool/Toddler Room:No bleach solution present for disinfection.

      • Incomplete cleaning checklists in some rooms. Cleaning checklists and schedules must be routinely filled out to ensure regular cleaning and disinfection

      • Kinder Room: 2 Lego table tops observed with brown sticky material
