      • 1. The product obtained for skin preparation prior to waxing services (Satin Smooth Azukene Oil) does not include a claim for cleansing the skin.Obtain a product that is specifically labelled for cleansing the skin. Update the written procedures to reflect the new product.       • The SDS sheet provided for the nail polishes only listed chemical hazard information and did not include the complete cosmetic ingredient list. Provide a complete list of ingredients for the nail polish or replace the products with ones that have complete ingredient disclosure.Previous violation:There were several brands of nail polish with no list ingredient.Obtain ingredient lists for the products or remove them from use.

    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • There were no facility-specific procedures for cleansing of the skin prior to waxing services and responding to accidental cuts and punctures.Develop facility-specific procedures using the template provided during the inspection, and ensure staff follow them.
    • 06. Do operators and personal services workers comply with the Personal Services Regulation and Standards and do they have the skills and knowledge to perform their duties in a safe and sanitary manner?
      • 1. Staff were unable to demonstrate proper knowledge of required disinfection practices for semi-critical manicure and pedicure tools. Develop and submit a written procedure outlining the proper cleaning and disinfection of all semi-critical tools, in accordance with the disinfectant manufacturer’s instructions for use.
    • 09. Is a point of service risk assessment being performed of the condition of the client and of the equipment to be used on the client prior to performing a personal service?
      • Staff informed the inspector that a nail care oil making claims for use on black nails, cuticles, and nail repair was being offered to clients with nail concerns, including possible infections.The nail oil was discarded onsite. Discontinue offering nail care oil or treatments to clients with nail concerns or possible infections. Refer clients with suspected nail infections to a healthcare professional.

    • 15. Is reusable equipment handled and stored properly (Critical)?
      • 1. Personal items and tools such as used toe separators, buffers and single use nail files were stored in the drawers of the manicure workstation.Remove all personal items from the manicure workstation. Ensure only properly cleaned, disinfected, and service-related tools are stored in this area.2. Clean semi-critical manicure and pedicure tools were stored on a dirty tray with visible debris observed.Clean and disinfect the tools and storage tray. Ensure to follow proper disinfection procedures for semi-critical instruments.
    • 16. Is reusable equipment handled and stored properly (Non-Critical)?
      • The linen used on the treatment bed in the waxing room was only being laundered at the end of the day. All linens must be removed and laundered after each client. Only clean linens must be used between clients.
    • 20. Is reusable equipment disinfected properly?
      • 1.The high-level disinfectant, preempt CS20 expired in November 2024. Discard the expired disinfectant and obtain a high-level disinfectant with a Drug Identification Number (DIN) or Medical Device License (MDL), labelled with the words: sporicidal or high-level disinfectant and TB, Tuberculocidal or Mycobacterium. 2. Semi-critical manicure and pedicure tools including, but not limited to, cuticle pushers, metal foot file, nail nippers, and clippers were being disinfected using alcohol or bleach.Discontinue the use or bleach or alcohol for disinfecting semi-critical tools. Ensure all semi-critical tools are reprocessed using an approved high-level disinfectant. 3. The hydrogen peroxide test strip used to verify the concentration of the high-level disinfectant was expired, indicating that the poured solution is being reused. Records were not being maintainedDiscard the expired test strips and obtain new hydrogen peroxide test strips. Verify and record the disinfectant concentration prior to reuse.4. The container identified for disinfecting the foot file did not allow for full submersion of the tool.Obtain a properly sized, cleanable container that allows the foot file to be fully submerged during high-level disinfection.5. The intermediate-level disinfectant, preempt RTU solution and wipes, expired in 2022.Discard the expired disinfectants and obtain an intermediate-level disinfectant with a Drug Identification Number (DIN) or Medical Device License (MDL), labelled with the words: disinfectant and TB, Tuberculocidal or Mycobacterium. 6. The pedicure foot bath was being disinfected with a mixture of expired preempt CS20 and Lysol toilet bowl cleaner.7. Discontinue the use of the unsafe mixture of Lysol toilet bowl cleaner and expired Preempt CS20. Use an approved intermediate-level for the pedicure foot bath.

      • 1. There were several brands of nail polish with no list ingredient.Obtain ingredient lists for the products or remove them from use.2. There were several unlabelled products observed.Label all products to clearly identify their contents.

      • A reusable scoop was stored directly inside the scrub container. The scoop was removed from the container during inspection. Clean and disinfect reusable scoop after use to prevent contamination of the bulk supply.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • 1. Several areas throughout the facility were unclean, with visible dust and debris observed in the following areas, including but not limited to:- Workstation drawers, tables, and storage containers- Areas behind the pedicure chairs- The back storage/ staff area- The area under the handwashing sink in the service area- The area around the laundry roomDeep clean the entire facility2. The back storage area next to the reprocessing sink was disorganized.Reorganize the back storage area to ensure all items are stored in a clean, orderly, and sanitary manner.3. Wax residue was observed on the surface of the wax warmer.Clean and disinfect the wax warmer.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • Staff food and personal items were stored in close proximity to the reprocessing sink.Remove all food from the reprocessing area and ensure the workspace is used only for storing and preparing tools for reprocessing.

    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • Facility was open to the public but offering limited services such as manicure during a water outage. Running hot and cold water is required in order to operate a personal services establishment safely. Tool cleaning, disinfection, hand washing, and sanitation of surfaces cannot occur safely if for any reason, facility is not supplied with running hot and or cold water.
