      • Observation(s): evidence of rodent activity noted in the niche ( on the wall behind the electrical panel cover; feces found on top of the insulation) holes on the wall ( 2, square shaped openings-first one underneath the three comp't sink while the second one is behind the three door cooler) must be covered/sealed to plug entry points for pests

      • Observation(s): leftover flour in batter station bin must be discarded ( bin to be emptied, then washed and sanitized prior to refilling with flour) storage shelf under the counter (front) must be organized washed and sanitized to ensure disposable food contact surfaces ( cups and/or spoons) are maintained clean pull out drawers under the S/S table must be organized and cleaned/sanitized- non food items must be kept separate from food contact surfaces ( eg, knives, tongs, etc) three compartment sink must be scrubbed clean and sanitized

      • Observation(s): F2.3B Pans (7) for the hot holding case ( containers for fried chicken and fried potatoes) haven't been washed/sanitized since the closure of the Chester's operation; the interior surfaces of the microwave must be wiped clean and sanitized as well. Operator must ensure pans, racks and the microwave are washed and sanitized prior to resuming food service operation in the premises.
      • F2.3B Manual dishwashing procedure must provide sufficient washing and sanitizing action to remove contamination.
    • F2.5 Are the premises and equipment properly maintained and operated in a sanitary manner (i.e. ongoing maintenance)?

      • Observation(s): chicken tenders being displayed in the hot holding case was observed with an Int temp of 55°C ( chicken leg piece was probed /checked and observed temp was 61°) single serve (individual portions) of Dairyland dairy milker (2%) and Dairyland dairy creamer (18%) were being stored at ambient temps on the coffee counter Walk in cooler ( for washed raw chickens) , Cocal cola fridge ( raw chicken in the back), True Refrigerator for boxed draw chickens in the back were checked and observed keeping dressed chicken at safe cold temps.

      • Observation(s): no running hot water in premises ( employee stated there was a fire in the back bldg attached to the store and it affected the hot water supply in the bldg)

      • Observation(s): un sanitized, damaged tong being used to dispense cooked PHFs from hot hold case; Ready-to-eat cookies on the coffee counter were being displayed naked) ie not behind sneeze guard or individually wrapped to preclude contamination)

      • Observation(s): The permit is hidden in the corner of the wall. Karandeep moved it out from its hiding place, so now it's in a conspicuous location. The permit is for Scoop ice cream ( other menu items have been discontinued since last inspection-see comments under F1.1 above)

      • Observation(s): Chicken pc on the lower shelf was checked ( observed int temp was 61°C), chicken tender on the upper shelf was also checked ( observed int temp was 58°C)
      • F1.2A Potentially hazardous foods must be stored or displayed at a temperature of 60 °C (140 °F) or hotter.
    • F1.5 Has the operator taken steps to ensure employees are using adequate hygiene measures to prevent contamination of food?
      • Observation(s): Interview with employee on where he washes his hands reveals that the expectation for double step handwashing ( after using the washroom) requirement is not being met ( he said, he washes hands inside the washroom, then uses Hand sanitizer once he steps into the kitchen/food prep area prior to performing food handling tasks.
      • F1.5A Employees must wash their hands as often as necessary to prevent the contamination of food.
      • F1.5B Employees must exhibit cleanliness and good personal hygiene.

      • Observation(s): Sanitizer solution in spray bottle did not contain adequate concentration of Chlorine and very little volume ( <1 L of prepared solution) is remaining in the bottle; no spare bottle of undiluted bleach in premises. Operator must ensure an ample supply of sanitizer is maintained onsite to allow sufficient sanitizing of food contact surfaces and frequently touched items.

      • Observation(s): product inside hand soap dispenser in food prep area dedicated hand wash station is questionable ( poor quality soap/ soap that's been watered down, employee thinks it's hand sanitizer (?) and not foam soap)

      • Observation(s): Observation of level of cleanliness/sanitation in frequently used utensils ( ice cream scoops, tongs for hot holding case) points to lack of compliance with written cleaning and sanitizing procedures ( Sanitation Plan)- in that frequency ( "after every use") stated on the plan is not being followed. Discussed Dipper Well Policy and alternatives with Remi and she agreed to revise Sanitation Plan to reflect what can be met operationally that will fall within acceptable Critical Limits of Sanitation Plan and Food Safety Plan.

      • Observation(s): Natrel 10% dairy creamer ( single portions) is kept at ambient temperatures ( not inside fridge and no ice cubes or ice packs in the container for the creamer)-CDI; Merco hot holding unit keeps fried foods at temperatures ≥60° C ( fried chicken was probed 30 minutes after being placed into the hot holding case- actual temp observed was 68°C)

      • Observation(s): food handler washed her hands without applying soap, she then proceeded to don disposable gloves prior to handling food. EHO had to remind food handler of hand washing steps that must be taken every time hands are washed.
      • F1.5A Employees must wash their hands as often as necessary to prevent the contamination of food.

      • Observation(s): utensils ( tongs and ice cream scoop) after being washed were kept on the edge of the two compartment sink ( after air-drying, utensils must be stored inside the drawer to keep food contact surfaces in a sanitized condition and to protect from contamination); Tongs for fried chicken in the hot holding unit must be kept in a cleanable tray or pan ( instead of on top of counter to protect from contamination) EHO had to remind the food handler of the proper warewashing steps ( wash, rinse, sanitize, and air dry) and the expected frequency for washing and sanitizing of utensils ( after every use)

      • Observation(s): Waring Big Freeze screamer mixer and Electro Freeze soft serve machine must be added to the establishment's Sanitation Plan ( soft serve machine hopper must be sanitized once every 24 hours). EHO requests to see Operator's Manual for Electro freeze machine.

      • Observation(s): 1 ice cream scoop observed being stored at ambient temperatures inside disposable cup with water-this doesn't comply with ICE CREAM SCOOP POLICY-suggested to Jaspreet to store clean scoops inside ice cream freezer and have at least 3 scoops on hand ( 1 for ice cream without nuts, another one for ice cream with tree nuts, and a third for ice cream with peanuts)

      • Observation(s): Potato cutter backboard is constructed of raw wood ( not smooth, not cleanable) so this must be replaced with a material that is smooth and cleanable. The bottom catchment area of potato board was observed to be filthy ( dried up leftover potato bits)-this space must be modified to ensure surfaces are easily cleaned and routine cleaning schedule is maintained to prevent build up of filth.
      • F2.6B All food contact surfaces must be constructed from materials that are suited for their intended purpose, durable and free from any noxious (harmful) or toxic substance.

      • Observation(s): Three employees were working the shift at the time of Inspection (Jaspreet Singh, Sukh, and Harpreet). None of the three holds a valid current FS1 certificate.
      • F3.6A Operator must have valid FOODSAFE certificate or equivalent training.
      • F3.6B In the absence of the operator, at least one employee present in the establishment must have valid FOODSAFE certificate or equivalent training.

      • Observation(s): No Sanitation Plan at time of Inspection EHO unable to verify if food contact surfaces/food storage equipment have been sanitized prior to starting illegal food service operation ( June 7th was when they started)
    • F3.1 Does the operator have a valid operating permit or approval to operate?
      • Observation(s): Application form filled out at time of Inspection; Operator agrees to STOP serving hot pizza and not to start serving scoop ice-cream until PERMIT TO OPERATE has been given
