    • F2.2 Are proper pest control measures in place on the premises?
      • Observation(s): Observed a deceased mouse near the back emergency exit. Operator stated that a pest control company services the facility once every three weeks and that additional in-house pest control measures are also in place. EHO discussed the following with the operator: - All food items must be stored at least 6 inches off the floor. - Clutter (e.g., excess cardboard, wood) should be minimized to reduce harbourage conditions. - The back door must be fully sealed with weather stripping. - Any food products that have been accessed by pests must be discarded. - Dead pests must be removed immediately, and the affected area must be cleaned, rinsed, and sanitized with a 200ppm bleach water solution. Facility is to provide the most recent pest control service report.

      • F2.2C The premises must be maintained free of pests.

    • F2.2 Are proper pest control measures in place on the premises?
      • Observation(s): Observed holes in walls near front and back doors, these openings may allow for entry or harborage of pests and must be patched. Contaminated/outdated products observed stored in the loading bay, these should be removed from the premises to help maintain pest free facility.
    • F2.5 Are the premises and equipment properly maintained and operated in a sanitary manner (i.e. ongoing maintenance)?
      • Observation(s): Ceiling is damaged and in disrepair. Overhead drywall sheets are cracked/broken and dangling, some panels appear to be water damaged and may have mold growth on them. Damaged ceiling sheets could cause serious physical injury if debris falls while staff are underneath. Falling debris may also lead to contamination of products in storage.
      • F2.5B The premises must be properly constructed and maintained in good, clean and sanitary condition.

    • F1.4 - Are proper refrigeration, freezing and thawing procedures followed?
      • Observation(s): Daily logs started, however no records recorded this week. March 13, 14, 15,
    • F2.2 - Is the premises free from a pest infestation?
      • Observation(s): The flooring in the trailer, will require a detailed cleaning along the wall and between the rows in the floor of the trailer. A vacuum and an extension cord is needed for staff to clean. There is metal and old parts and wooden debris, papers, etc, stored on the outside wall of the warehouse and the trailer. Keep copy of pest control operators note and dates of visits at the site.
      • F2.2A - The premises must be maintained free of pests.
    • F3.3 - Is the premises construction and layout unchanged and in accordance with approved plans?
      • Observation(s): Provide EHO with a date that roof should be repaired by. Ceiling is water damaged.
    • F3.5 - Are there handwashing stations available and properly supplied?
      • Observation(s): There is no washroom or hot water in this building for staff. Informed that washroom in adjacent building is no longer made available for staff. Require hot water for had washing and cleaning, and a washroom for the staff.
      • F3.5A - Handwashing station must be supplied with hot and cold running water.
      • F3.5B - Handwashing station must be supplied with liquid soap in a dispenser.
      • F3.5C - Handwashing station must be supplied with single-service product for drying hands.
      • F3.5D - Handwashing stations must be maintained adequate in number and location to ensure convenient access to all employees.

    • F1.4 - Are proper refrigeration, freezing and thawing procedures followed?
      • Observation(s): No record logs of temperature in cooler or freezer. Require daily logs be kept, and recorded.
    • F2.2 - Is the premises free from a pest infestation?
      • Observation(s): Rodent dropping noted by storage of Pizzeria flour. Clean area. Insect and rodent dropping noted in the trailer. The flooring in the trailer, will require a detailed cleaning along the wall and between the rows in the floor of the trailer. Clean and remove pallets, wooden debris, papers, etc, stored on the outside wall of the warehouse and the trailer. Keep copy of pest control operators note and dates of visits at the site.
      • F2.2A - The premises must be maintained free of pests.
    • F3.3 - Is the premises construction and layout unchanged and in accordance with approved plans?
      • Observation(s): Ceiling is water damaged, water is leaking into the warehouse in a couple of areas. The ceiling in need of repair at these water damaged areas. Water is leaking into the warehouse at time of inspection.
