      • At the time of the inspection spray bottles containing chlorine solution only measured 100 ppm.This was corrected by increasing chlorine concentration to 1000ppm bleach.

      • AHS is to be contacted following the outbreak guidelines for childcare operators when there are two or more clients or staff exhibiting GI symptoms within 48 hours of each other. This also would include tracking of clients symptoms and reasons for absences.

      • Ensure clients exhibiting symptoms do not return unless cleared. During normal operation, clients and staff exhibiting symptoms are not to return until 48 hours symptom free.

      • Some of the baskets and containers in the facility are not constructed of material that are smooth, impervious to moisture and easy to clean.Ensure that all containers, equipment, and surfaces are smooth and easy to clean, or can be laundered on a routine basis.

      • Plush items, including but not limited to; dress up clothes, stuffed animals, chairs, carpets, and covers should be laundered on high heat daily for remainder of period. If this is not available to occur, remove from use until time period is over.

      • Some of the art supplies such as glue, glue sticks and bingo dabbers were not labeled as non-toxic.Ensure that are supplies being used by clients are non-toxic.

      • The hand sink in the baby room was not draining properly.The hand sink in the Kinder washroom was not draining properly. Ensure that plumbing is functioning as designed.

      • * Three electrical outlets at the OSC room was missing a protective cover/cap. Please ensure all electrical outlets at the facility are supplied with protective cover/cap at all times. *Safety lock for the fridge in the baby room was not in place. Please correct.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • *First aid kit was missing band aids. The kit had only very small dot like first aid band aids. Please ensure first aid kits up to date.

      • *One burnt light observed in the washroom. Please ensure adequate lighting is available for sanitation.

      • * Several vent covers were observed to be needing cleaning. Dirt and debris were observed on the vent. Please ensure all vent covers at the facility are cleaned on a routine basis.*Extra cleaning in hard-to-reach areas (behind sensory table, storage shelves) was observed to be needed.*Ceiling tile in OSC room in disrepair. Ensure all surfaces are easily cleanable and impervious to moisture.

      • Safety belt for the diapering area in the baby room appears to be missing. Ensure a safety belt is installed immediately.

      • *Wall behind the hand sink was observed to be peeling. Exposed drywall observed. Ensure all areas are easy to clean and impervious to moisture.*Carpet in front of the kitchen door appears to be in disrepair and fiber appears to be coming undone. Ensure flooring is in good repair. At this time a mat was placed on top.

      • *Safety lock for the tall cabinet is not available. Ensure safety lock is in place (bug spray and lotion stored in the cabinet).

      • *Safety belt for the highchair was missing. It appears the safety belt was removed temporarily for cleaning.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • *Wall behind the hand sink was observed to be peeling. Exposed drywall observed. Ensure all areas are easy to clean and impervious to moisture.*Carpet in front of the kitchen door appears to be in disrepair and fiber appears to be coming undone. Ensure flooring is in good repair.*Ceiling tiles in the two OSC bathrooms appeared to be in disrepair. Ensure ceiling tiles are in good repair.

      • *Sink appeared to have draining issues in the infant room. Water appears to not be draining.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • *What appeared to be clean cloths were stored on the toilet bowl (on the holding tank). Ensure cloths are kept away from contamination. Paper towel stored in a dispenser is recommended for the area.*What appears to be a basket with clean cloths were stored on top of a mop bucket. Ensure clean cloths are adequately protected from contamination.*Paper towel roll was observed to be stored on the diapering table. Ensure paper towel is protected from contamination.*Ensure sanitizer bottles are labeled to show contents. Various bottles were observed for bleach solutions (i.e. purexall, clear containers etc)

      • * The door leading to the utility/mechanical room in the Kinder room was easily accessible. Items such as hot water tank and dishwasher are located in this room.Please install a safety locking device for the door.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • * Hot water temperature from the Toddler room washroom handwashing sink faucet was measured to be at 56C. Please reduce the hot water temperature at the facility. Maximum of 54C is required, 49C maximum recommended. Oct 10, 2023: The hot water temperature in the rooms was measured to be at 57C. Please note that this is an ongoing critical violation and must be corrected immediately.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • * Sleeping mats with sheets in the baby room were stored on top of each other without separation. The mats are not disinfected daily after use. Please create a separation between the mats.

      • * Personal items (such as clothing's) stored on hooks in the pre-school room were touching each other. Please create a separation between personal items for each individual child.

      • * An electrical outlet at the OSC room was missing a protective cover/cap. The staff replaced the missing cover during inspection.Please ensure all electrical outlets at the facility are supplied with protective cover/cap at all times.

      • * The door leading to the utility/mechanical room in the Kinder room was easily accessible. Items such as hot water tank and dishwasher are located in this room.Please install a safety locking device for the door.

      • * Less than less than 100ppm and 10ppm bleach solution was measured from spray bottles in the baby room and little preschool room diapering areas. Please increase the concentration of bleach disinfectant. A minimum of 100 ppm chlorine solution is required for normal operations (when not in outbreak).

      • * The vent cover in the Toddler washroom was unclean. Dirt and debris were observed on the vent. Please ensure all vent covers at the facility are cleaned on a routine basis.

      • * Hot water temperature from the Toddler room washroom handwashing sink faucet was measured to be at 56C. Please reduce the hot water temperature at the facility. Maximum of 54C is required, 49C maximum recommended.

      • * Push pins and staple pins were being used to hold artwork, paperwork and other items on boards in some rooms (baby and toddler rooms). Please remove the push pins, use a safe means of holding paperwork or artwork and cover the stable pins with a clear tape to prevent them from falling off. The pins can fall off and get swallowed by a child.
