      • OBSERVED THE EXIT DOOR NEAR THE LAUNDRY ROOM IN THE BASEMENT WITH A GAP OF ABOUT 1/4 OF AN INCH. INSTRUCTED TO SEAL AS A MEANS OF PREVENTING POSSIBLE PEST ACTIVITY.

      • OBSERVED THE STEAM TABLE ON THE THIRD FLOOR RUSTING AND MISSING SNEEZE GUARD ON THE LEFT SIDE AND THE RIGHT SIDE IS NOT EASY TO CLEAN AND MAINTAIN...ALSO OBSERVED THE STEAM TABLE ON THE SECOND FLOOR WITHOUT SNEEZE GUARDS...ALSO THE FIRST FLOOR WITH SNEEZE GUARDS. INSTRUCTED TO PROVIDE FOR ALL AND MAINTAIN AS A MEANS OF PREVENTING POSSIBLE CONTAMINATION OF FOODS DURING HOLDING FOR SERVING.

      • OBSERVED THE HAND WASHING SINK IN THE WOMENS TOILET ROOM ON THE FIRST FLOOR COMING LOOSE FROM THE WALL (SEAL BREAKING) INSTRUCTED TO REPAIR AND MAINTAIN.

      • OBSERVED THE FLOOR PANEL IN THE WALK IN FREEZING UNIT COMING LOOSE FROM THE FLOOR CAUSING POTENTIAL TRIP HAZZARD AS WELL AS MAKING AREA NOT EASY TO CLEAN AND MAINTAIN. INSTRUCTED TO REPAIR AND MAINTAIN.

      • OBSERVED A LEAK AT THE DRAIN PIPE OF THE SANITIZING SINK AND OBSERVED THE SPRAYER HEAD AT THE DISH WASHING MACHINE LEAKING. ALSO OBSERVED THE BACKFLOW DEVICE AT THE TOP OF THE DISH WASHING MACHINE. INSTRUCTED TO REPAIR AND MAINTAIN.

      • OBSERVED ALL WOMENS TOILET ROOMS ON EACH FLOOR THAT THE STAFF MAY USE WITHOUT TRASH RECEPTACLE WITH LID. INSTRUCTED TO PROVIDE FOR ALL WOMEN TOILET ROOMS.

      • OBSERVED THE LOWER WALL ACROSS FROM THE DISH WASHING MACHINE DAMAGED AND IN NEED OF REPAIR. INSTRUCTED TO REPAIR AND MAINTAIN AS A MEANS OF PREVENTING POSSIBLE PEST AND INSECT ACTIVITY AS WELL AS MAKING AREA EASY TO CLEAN AND MAINTAIN.

      • OBSERVED PEELING PAINT ON THE CEILINGS THROUGHOUT FOOD PREP AREA AND DISH WASHING AREA. INSTRUCTED TO REPAIR AND MAINTAIN AS A MEANS OF PREVENTING POSSIBLE CONTAMINATION OF DISHES AND FOODS PREPARED IN AREAS.

      • OBSERVED INSIDE THE CABINETS IN THE SMALLER DINING AREAS ON THE FIRST AND SECOND FLOOR WITH WHAT APPEARS TO BE BLACK MOLD, DIRT AND DEBRIS AND DAMAGED EDGES. INSTRUCTED TO CLEAN, REPAIR AND MAINTAIN ALL.
