      • In food establishments, there shall be adequate sewage and waste water disposal facilities that comply with all the requirements of the plumbing section of the Municipal Code of Chicago. WASTE WATER RUNNING ONTO FLOOR UNDER BAR THREE COMPARTMENT SINK....VERY BAD LEAK FROM PIPES UNDER SINK.

FOUND APPROX 20 FRUIT FLIES AND 15 MICE DROPPINGS ON PREMISES. ALSO FRONT DOOR NOT RODENT PROOF. NO PEST CONTROL LOG BOOK ON PREMISES.
