      • OBSERVED THE FRONT PREP AREA FLOOR TILES MISSING GROUT, FRONT PREP AREA WITH RAW CONCRETE AROUND THE FLOOR DRAIN, FRONT PREP AREA FLOOR DRAIN WITH A DRAIN COVER IN DISREPAIR, BROKEN FLOOR TILES AT THE PREP AREA DOOR THRESHOLD AND ACCUMULATED GREASE ON THE FLOOR BEHIND THE COOKLINE. INSTRUCTED TO PROVIDE AND MAINTAIN FLOOR GROUT BETWEEN THE FLOOR TILES, PROVIDE AND MAINTAIN FLOOR TILES IN NOTED RAW CONCRETE AND BROKEN FLOOR TILE AREAS, REPLACE THE FLOOR DRAIN COVER AND CLEAN AND MAINTAIN THE FLOORS.

      • OBSERVED BROKEN WALL TILES BEHIND THE GYRO COOKER, MISSING WALL TILES AT THE RANGE, HOLES IN THE WALL NEXT TO THE PREP AREA DOOR AND ACCUMULATED GREASE ON THE WALL BEHIND THE GYRO COOKER. ALSO OBSERVED ACCUMULATED DUST ON LIGHT SHIELDS I THE PREP AREA. INSTRUCTED TO PROVIDE AND MAINTAIN WALL TILES AND CLEAN ALL WALLS AND CLEAN ALL LIGHT SHIELDS.

      • OBSERVED THE 3 COMPARTMENT FAUCET BASE LEAKING, NO DRAIN STOPPERS FOR THE 3 COMPARTMENT SINK, THE 1 COMPARTMENT FAUCET MISSING AND NO MOP SINK ON THE PREMISES. INSTRUCTED TO REPAIR AND MAINTAIN ALL PLUMBING, PROVIDE 3 DRAIN STOPPERS, PROVIDE A FAUCET AT THE 1 COMPARTMENT SINK AND PROVIDE A MOP SINK.
