      • There is a tall white shelf (called the sensory shelf) in the 3-5 year old room that is not secured/mounted to the wall.Please ensure all tall shelving is properly secured or mounted to the wall to prevent any tipping hazards.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • There was as damaged pretend food toy in the 16 month to 3 year rold room.The operator immediately discarded the toy.

      • - Paint glitters, labeled for children over 5, were found in the classroom of 3-5 years ols.*Staff discarded the glitters and will replace them with non-toxic, age-appropriate art materials.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. The top surface of the red table in the main room has started to peel off.*Please ensure that all surfaces are smooth, non-absorbent, and easily cleanable.2. The caulking around the Montessori washroom sink has started to peel off, and the sink coating appears to be damaged.*Replace or repair the sink and recaulk around it to ensure proper sanitization and cleaning.

      • The lid of the water table container had accumulated dust and sand.*Staff cleaned the lid. Ensure all equipment is maintained in a clean and sanitary condition.
    • 31. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Critical)?
      • - A fire extinguisher was found on the floor of the Montessori room bathroom, and the pin was not secured.*Ensure that the fire extinguisher pin is properly secured and kept out of the reach of children. The violation was corrected onsite by staff.

      • - The foot pedal on the diaper bin in the Main Room washroom was non-functional.*Please repair or replace the bin with a fully functional lid to ensure the proper hygiene.

      • Some holes and gaps were observed in the daycare facility. -Please seal all holes/gaps/crevices to prevent pest entry.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • There was no caulking around the hand washing sink in baby room.-Please apply silicon sealant to the indicated area to prevent water infiltration.
    • 26. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Critical)?
      • Fresh mouse droppings were observed in the montessori room.-Please consult with a pest control expert to identify and seal all potential pest entry points, such as holes, gaps, or crevices, in the daycare facility.-Clean and disinfect the affected areas using a freshly prepared bleach solution with a concentration of 1000 ppm.

      • Mouse droppings were observed in the montessori and upstairs rooms, as well as under the cabinet in the upstairs room.-Please consult with a pest control expert to identify and seal all potential pest entry points, such as holes, gaps, or crevices, in the daycare facility.-Clean and disinfect the affected areas using a freshly prepared bleach solution with a concentration of 1000 ppm.

      • Peeling paint was observed on the wall beside the washroom in the kindergarten classroom.-Please repaint the indicated area and maintain in good repair.

      • 1. Glass mirrors were observed within children' reach in baby, young 3's and young toddler's rooms. -Please ensure all the mirrors with children's reach are shatterproof to prevent injury.The mirrors were removed during the inspection.2. Long Tangling wires were observed in kindergarten and before and after school care rooms.-Please ensure all long tangling wires/ropes are kept out of reach of children to prevent risk of entanglement.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • 1. First aid kits in baby and Young 3' rooms did not have approved antiseptic wipes. -Please equip the First aid kit with suitable antiseptic wipes/moist wipes.2. Some materials in the First aid kit in baby's room was damaged/torn and the content have spilled all over the first aid bag.-Please replace the damaged First aid materials and clean the first aid bag.-Also provide CPR mouthpiece in the First aid kit.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. A hole was observed beside the back door in baby's room.-Please repair the indicated area to prevent entry of pest.2. Dirty diapering mat was noted in baby's room.-Please ensure the diapering surface and its surrounding area are cleaned and sanitized after each diapering process.3. Damaged and dirty faucet was noted in Young 3's and toddler's rooms.-Please repair and clean the faucet and areas around the handwashing sink to maintain sanitary conditions.4. Worn caulking was observed around and behind the handwashing sink in the before and after school care.-Please apply new sealant around the indicated sink areas to prevent water infiltration.

      • Dirty toy containers were observed in the before and after school care room.-Please clean and sanitize the indicated containers and ensure they are in sanitary condition at all times.

      • Kindergarten room: Mirror was missing frame at the bottom, exposing sharp edges of the mirror. Fix mirror.
    • 10. Is a properly supplied first aid kit provided and readily available?
      • First aid kits throughout facility do not have approved antiseptic wipes/alcohol wipes. Source alcohol wipes with "DIN" or "NPN" number, and stock all first aid kits.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • BASC basement bathroom wall behind toilet is damaged. Refinish and repaint wall so that it is smooth, easily cleanable, and impervious to moisture.
