      • single use items must be thrown out after use, do not put them in the drawers after use, this shows single use items being reused.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Evidence of inadequate cleaning of equipment observed at workstations. Ensure all semi-critical tools are cleaned and disinfected with CS20 prior to reuse. This includes the drill bits.
    • 20. Is reusable equipment disinfected properly?
      • High level disinfectant logs must be maintained and tested. PHI will send log sheet; all information must be documented. CS20 must have a contact time of 20 minutes.

    • 20. Is reusable equipment disinfected properly?
      • 1. High-level disinfectant, preempt CS20, was not being used in accordance with manufacturer's instructions, equipment such as nail clippers or cuticle cutters were only dipped into CS20 solution for few seconds.-Please use the high-level disinfectant following manufacturer's instructions. Clean, rinse and dry the indicated equipment before fully immerse in the CS20 solution for 20 minutes, follow by drying and packaging.2. The high-level disinfectant was being reused for more than one day without the required testing with suitable test strip.-Please procure high-level disinfectant's test strip and ensure its concentration is being monitored for the re-use period.Also, ensure to document the high-level disinfectant test result.3. Alchol used to disinfect non-critical equipment such as brow scissor and tweezer.-Please use disinfectant such as Mr. Clean to disinfect such equipment after cleaning step.

      • Overly filled sharp container was noted during the inspection.-Please properly dispose of the indicated waste container through the appropriate medium as needed. The waste must not be filled past the line provided on the sharp container .
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Cosmetic container without the label was observed during the inspection.-Please label all cosmetic containers with product names to prevent the risk of misuse.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Slightly worn wax treatment chair was noted onsite.-Please repair to ensure proper cleaning of surfaces.
