      • Facility was using the tools reprocessing sink for the hand washing, however written procedures for sink decontamination between use as handwashing and tools reprocessing were not available.Template would be provided with this e-mail.Please fill the template and send the picture to PHI.

      • High level disinfectant was being used for 14 days, as per manufacturer instructions, however HLD logs were not maintained.Please make sure HLD is tested on routine basis and logs are maintained.Template for HLD logs would be provided with this e-mail.

      • The operator uses CS-20 to disinfect semi-critical equipment.However, there are no test strips at the facility and high-level disinfection logs were not being maintained. A high-level disinfection log was provided to the operator. Please purchase CS-20 test strips and maintain disinfection logs moving forward.
