- 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
- Written procedures were not available for cleansing of the skin and mucous membranes and client care in the event of accidental skin cut or puncture. - Ensure required written procedures are completed and available onsite. Written procedures were completed during the inspection.
- 14. Is single-use or uncleanable equipment being used as intended (Non-Critical)?
- A wooden board wrapped in masking tape with visible eyelash glue debris on the surface was noted to be stored in the right treatment room station drawer.- Ensure equipment that is used for services that contact the mucous membranes, such as eyelash extensions, can be properly cleaned and disinfected. The operator reported this board was used for "practice only" and removed the board from use.

- Nail brushes and tweezers with visible debris were stored with clean equipment at a manicure station. - Ensure all used equipment is separated from clean equipment and moved to the reprocessing area. The operator removed the brushes and tweezers to be reprocessed.
- 20. Is reusable equipment disinfected properly?
- The operator reported the UV sterilizer was used for disinfecting semi-critical equipment. High level disinfectant was not available at the time of inspection. - Ensure semi-critical equipment is disinfected with a high level disinfectant at minimum. High level disinfectant (PREepmt CS20) was obtained by the manager during the inspection to disinfect equipment. Disinfectant containers are to have lids and be deep enough to completely soak equipment. No high level disinfectant test strips were available at the time of inspection. - Ensure appropriate high level disinfectant test strips are available to test the high level disinfectant as per the manufacturer's instructions. The manager obtained high level disinfectant test strips (Virox APH 7.0%) during the inspection. Test strips are to be marked with the date of opening and be discarded after 3 months as per manufacturer's instructions. High level disinfectant records were not available during the inspection. - Ensure accurate high level disinfectant records are maintained and available onsite. The manager completed daily high level disinfectant log sheet during the inspection.Metal manicure drill bits were soaking in liquid reported to be alcohol solution as per operator. - Ensure all semi-critical equipment is cleaned and then disinfected with a high level disinfectant at minimum. The operator removed the drills bits and relocated them to the reprocessing area to be cleaned and then disinfected.

- An open plastic container was noted to be full of sharps with no lid. - Ensure the sharps container has a tight fitting lid and is not overfilled.

- Various nail polish containers, a bottle of "eyelash glue", and eyelash bonding with inadequate labelling were noted in the facility that lacked information such as the product identify, manufacturer information, list of ingredients, and/or manufacturer's instructions. - Ensure all cosmetic products are labelled properly with the product identity and manufacturer information with ingredients and manufacturer's instructions for use information available. The operator removed products for personal use.

- Two wax pots were uncovered in the back left service room. - Ensure all cosmetic products stored in a sanitary manner and protected from contamination. The operator covered the wax pots.

- Various pedicure massage chairs had non-intact surfacing exposing absorbent material. - Ensure surfaces that may touch client's skin or be touched by the personal service worker are cleanable and able to be disinfected. Repair or refinish massage chair surfaces to allow for proper cleaning and disinfecting as needed. The operator reported new chair covers were already ordered.
