    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • *** Written procedures for skin cleansing, sink decontamination, and for accidental cuts and/or punctures were not available.Corrective Actions:- Establish facility specific written procedures for sink decontamination.- Establish facility specific written procedures for accidental cuts and/or punctures.- Establish facility specific written procedures for any steps taken for cleansing the client's skin prior to all services (pedicures, manicure, waxing, nails, etc.) Ensure these procedures are posted in an area that is visible and accessible to all staff.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • Hand washing sink was accessible, supplied with liquid soap and single use, laundered towels.*** Alcohol based hand rub (ABHR) was expired since 11/2008. Product was removed during the inspection.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • There was ample supply of single use, disposable buffing pads, emery boards, etc. Pedicure liners were used to reduce gross contamination of the basins, and to make cleaning and disinfection easier.*** Pedicure impellers are single use, disposable, however several units were installed on previously cleaned pedicure basins, on top of new pedicure basin liners. *** Single use disposable mascara wands were found in the waxing room.Actions Required:- Discard all previously used, single use, disposable items. The affected items were identified and removed during the inspection.
    • 20. Is reusable equipment disinfected properly?
      • Preempt CS20 (exp Feb 10, 2026) and Preempt RTU (exp Jan 2027) high level and intermediate level disinfectants were available.AHP 7 test strips were available.*** AHP 7 test strips were not used to test the efficacy of the disinfectant, nor were results being documented as required.Corrective Actions:- Follow the manufacturer's instructions for use for all disinfectants and test strips.- Recommend that a timer be used to ensure that equipment is soaked for the minimum amount of time (20mins) and not longer to prevent corrosion of equipment.- Preempt CS20 has a 14-day reuse period, and the efficacy must be tested each day the disinfectant is used.- Daily test results must be documented. Provide proof of documentation by January 12, 2026.
