- Waxing was being offered as a service. EPH was not notified prior to offering the new service. The personal services notification link was provided to the operator.

- One of the disinfectant containers with barbicide appeared cloudy and had physical contamination. Several tools were observed in the container. The operator stated that the station was not currently in use and the worker was currently away. A new solution was prepared upon request and the tools were disinfected.
