    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • Tall weeds are growing along the side of the building near the side door which can attract pests.*Remove weeds.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • Wall next to burger prep table is stained from grime. It appears as though the wall was not painted after mudding to provide a cleanable surface. The shelve fronts is missing arborite next to the burger prep table, exposing MDF. *Refinish the wall area next to the burger prep table to be smooth and cleanable. *Finish exposed MDF on shelving to be smooth and cleanable.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • The following areas requires attention:- The underside of the stainless steel table is sticky.- Buildup is starting on floor in a few areas: around floor vents and edges of tile.*Clean and sanitize the mentioned areas. Discussed obtaining a degreaser to assist with removing floor grime accumulation.

    • 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
      • Pizza dough pressing equipment contains residual grime. Operator is not aware of how to take it apart for cleaning. Check with the operating manual to determine how to take the equipment apart to clean and sanitize food contact surfaces. Equipment must be prior cleaned and sanitized before use.

The shelve fronts is missing arborite next to the burger prep table, exposing MDF. *Refinish the wall area next to the burger prep table to be smooth and cleanable. *Align stainless steel counter with cabinet base. *Finish exposed MDF on shelving to be smooth and cleanable.

      • The following areas requires attention:- The top of the chest freezer storing blenders and food equipment is accumulating grime under tape edges covering the freezer. - The underside of the stainless steel table is sticky.- Wooden knife rack is grimy. Recommended switching to magnetic knife holder.- Back wall has a discoloration and appears dusty.- High touch surfaces, handles, under coolers and the light switch to the walk-in cooler need attention. - Buildup is starting on floor in a few areas: around floor vents and edges of tile.*Clean and sanitize the mentioned areas. Discussed obtaining a degreaser to assist with removing floor grime accumulation.

      • Two sink plugs are missing for the three-compartment sink which does not provide the means to conduct manual dishwashing.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • Tile floor not finished in the location for the sandwich prep cooler. Cement subfloor is pitted and not cleanable. Baseboard is missing along wall behind sandwich prep cooler and into the dining room. Holes are noted in walls throughout kitchen where wall anchors are or where wall anchors used to be. Wall corner is lifting near the walk-in cooler. There are cracks in the wall around light fixtures in bathrooms and between the floor tile and wall behind the front counter. Portions of wall is missing on the back wall near the pizza oven and near the back hand sink. Kitchen counter to store bread is pitted and exposing MDF on counter edges. Shelf was cut with MDF edge exposed. Ceiling tile is not in place above the walk-in cooler. *Repair flooring to be smooth and cleanable.*Walls must be smooth and easily cleanable. Finish wall edge, remove wall anchors, and fill all holes and cracks. Repair wall next to pizza oven and hand sink. *Counters and shelves must be smooth and easily cleanable. Repair areas of exposed MDF.

      • Areas have not been thoroughly cleaned and sanitized: floors and baseboards throughout the kitchen, walk-in cooler shelving and floor, stainless steel counter edges, three-compartment sink, inside refrigeration units and cooler racks, light switches.
