- Pieces of bread intended for making breadcrumbs were placed in an uncovered container and stored above the ice machine overnight.All food must be protected at all times during storage, preparation, serving, and handling to prevent contamination.

- 1. Biofilm (brown slime) was observed on the inner surfaces of the ice machine. The operator must immediately conduct thorough cleaning and sanitization of the ice machine or discontinue use of the equipment until it has been properly cleaned and sanitized.2. Accumulation of dust was observed on the fan of the walk-in cooler. Conduct cleaning to affected area of the walk-in cooler.

- Unfinished wood surface is used to hold the microwave. The wood surface must be painted/sealed so that is easy to clean and impervious to moisture, or must be removed.
