    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1. Several staples used to secure the blue canopy in the Blue Room are loose and pose a potential hazard. Remove the staples and use an appropriate, secure method to attach the canopy.2. Sanitizer bottles measuring at 1000 ppm chlorine were found in the Blue and Green Rooms. Staff indicated this concentration is used for general cleaning and sanitizing of toys and diapering surfaces; however, this concentration is not appropriate for routine use and poses a potential health risk. 100 ppm is required for daily sanitizing, while 1000 ppm is intended for use during outbreak situations only. The 1000 ppm bleach was removed during inspection.

      • 1. During observation in the Blue Extension Room, proper diapering procedures were not followed. Gloves were not changed after handling a soiled diaper, and hand hygiene was not performed after sanitizing the surface until prompted. Ensure proper diapering procedures are followed, including changing gloves immediately after handling soiled materials and performing hand hygiene after each step. Gloves should be removed after handling the soiled diaper, followed by hand washing.

      • 1. Holes were noted in the wall in the Orange Extension Room. The wall must be repaired to maintain a smooth, cleanable surface.

      • 1. There are no designated bins for separating clean and soiled laundry; a single bin is currently used for transporting both. Provide separate, clearly labeled bins for clean and soiled laundry to prevent cross-contamination.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. A play floor mat in the Green Extension Room is damaged and not easily cleanable.The mat must be removed and replaced.2. The starfish-shaped pillow in the Orange Extension Room is torn with exposed filling. Please repair the pillow.3. Visible mold was noted on the washing machine gasket.The gasket must be cleaned and sanitized, and the door should be left open after use to prevent moisture buildup.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • 1. The sensory table in the Green Extension Room contained debris from a previous sensory activity and had not been cleaned and sanitized. Clean and sanitize the sensory table.2. The rolling pin used for playdough in the Blue Room had a build-up of debris and hair on the roller, and the storage bin was unsanitary. Clean and sanitize the rolling pin and storage bin.3. The plastic play blocks in the Blue Room had residual paint on some of the blocks.All blocks must be thoroughly cleaned and sanitized. Please use a brush if needed to clean crevices.4. A bolt was observed protruding from a dollhouse in the Orange Extension Room, posing a potential hazard. The bolt was secured during the inspection. Staff should be inspecting toys on a daily basis.

    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • The gasket in the washer is stained and moldy. Please clean and sanitize the gasket. Make sure the door is left open for the washer so it can be aired out between use.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • The staff washroom located adjacent to the laundry room is crowded with unused articles, cleaning equipment, paint can etc.Please remove some items and organize the room.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • Hand sinks in the classrooms and the new bathroom does not have paper towel dispensers to protect the bulk supply.Please install dispensers to prevent contamination of the bulk supply.
