      • Tall shelving units throughout facility were not adequately fastened to the wall. Anchor tall shelving units so that they do not pose a risk of falling into the children's play/nap area.

    • 06. Are proper protocols being followed in regards to client care?
      • - A toy tray was observed on top of the diapering mattress during the inspection.*Staff removed the tray from the area and sanitized both the mattress and the tray.**Ensure the diapering area is used solely for its intended purpose and not for storing items. This helps maintain proper hygiene and prevents cross-contamination. All surfaces in the diapering area should be kept clean, sanitized, and free from any non-related items to ensure a safe and sanitary environment for children.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • - Infant, Preschool, and Toddler (Main Floor): Duct tape residue remains on the floor where a rug was secured and needs to be removed to allow for easier floor cleaning.*Staff explained duct tape is in use to prevent slips, trips, and falls from carpet corners. Duct tape is not considered a smooth, easily cleanable, or non-absorbent surface.**Staff has been instructed to find an alternative method to secure the carpet and, in the meantime, ensure the floor is kept clean and free of any duct tape residue.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • - Staff winter jackets were stored above the children’s sleeping cots.*Staff removed the jackets and placed them in the staff room.**Ensure that personal items, including staff jackets, are stored separately from children’s sleeping areas to maintain a clean and safe environment. Personal belongings should not be stored above or near the cots to prevent any potential contamination

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Preschool room (upstairs) - one bottom cabinet below the staff sinks has a loose locking mechanism. Cabinet front was not locking- storage of chemicals noted inside. Repair locking mechanism or provide a child safe locking device to ensure no access by children.
    • 06. Are proper protocols being followed in regards to client care?
      • Toddler room- diapering table had some debris on countertop corners beneath the diapering pad. Ensure after diapering, the surfaces of the diapering pad and surrounding areas beneath the pad are cleaned and disinfected.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Preschool (main floor)- 1. Duct tape residue on floor where rug was formerly secured to requires removal, to allow for ease of floor cleaning. 2. One plastic storage bin for toys was cracked. Remove cracked bin from use. Toddler room- a few vinyl play mats have torn corner edges, open edges are no longer water proof and easy to clean. Replace the damaged vinyl sleeping mats.
