      • Observation: Walk-in freezer door does not close all the way. Air is expelling from the lack of seal. No issues with temperature control and food safety at this time. Manager expalins that they have put in a request for fixing the door
      • Corrective Action(s): Ensure the door can close flush to avoid risk of temperatures rising and the freezer malfunctioning.

      • Observation: Repeat violation. Flaking/peeling paint observed on pipes (ceiling and walls) in the walk in cooler. Spoke to area manager over the phone during the inspection. Area manager indicated that the issue has been looked at yesterday and will be completed by the end of next week.
      • Corrective Action(s): Repair flaking/peeling paint to protect food from contamination. Follow up with inspector once complete.

      • Observation (CORRECTED DURING INSPECTION): Hamburger patties in hot holding probed 44.8C. Hot holding unit was reported to have issues. Patties were made 30 minutes before the inspection.
      • Corrective Action(s): Operator will use up patties within the 2 hours - time tracking stickers placed on containers.
      • Use up all patties within 2 hours - discard any remainder.

      • Observation: (1) Hot holding unit for hamburger patties measured below 60C.
      • (2) Sink plugs missing at 3 compartment sink. Operator indicated that dishes are done at the end of the night and another manager will be bringing the sink plugs in the afternoon.
      • Corrective Action(s): (1) Repair/replace unit to opreate at 60C or hotter.

    • 205 - Cold potentially hazardous food stored/displayed above 4 °C. [s. 14(2)]
      • Observation (CORRECTED DURING INSPECTION): Photo evidence was collected for the violation noted below:
      • 1. 29 cartons of 237 mL 2% milk were noted with internal temperatures of 8.8C and 8.7C (two internal temperatures taken, one from the front of the stack and one from the back).
      • 2. A stainless steel container of individual creamer were noted with internal temperature of 13.8C.
      • Corrective Action(s): Note: Temperature of the refrigeration unit holding the milk and creamer was not taken this morning due to the power outage. As per the Manager, the unit was noted below 40F (around 35F) when it was checked yesterday. However, since we are not able to confirm how long the milk and creamer have been stored at above 4.0C, these items were discarded.
      • All 29 cartons of milk and creamer were discarded during the inspection - District inspector observed staff discarding each carton into the sink.

    • 308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
      • Observation: 1. The reach-in cooler holding the 29 cartons of milk and creamer (front near the cash register) was noted with an internal temperature of 9.5C.
      • 2. Leaking was observed from the fan unit in the walk-in cooler.
      • Corrective Action(s): 1. All cold potentially hazardous food (PHF) items were discarded from this cooler. Do not store any cold PHF in this unit until it is able to reach 4.0C or below. Manager thinks this unit was affected by the power outage this morning and had it reset once he was aware there was an issue with the unit. If the temperature does not come down to 4.0C within the next hour, please call to have this unit service.
      • 2. Fix the leaking noted in the walk-in cooler to avoid any of the dirty water from contaminating food stored within the unit.

      • Corrective Action(s): Repair the flaking/ peeling paint to avoid any pieces from coming off and accidentally becoming a physical hazard to food.
      • Clean and sanitize the debris noted on the ceiling of the walk-in cooler.

    • 302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
      • Observation (CORRECTED DURING INSPECTION): A couple of the sanitizer containers tested on the sandwich prep line were noted with 0 - 100 PPM QUAT. Wall mounted QUAT dispenser was noted with 300 - 400 PPM QUAT.
      • Corrective Action(s): **Issue corrected at the time of inspection** Shift Manager discarded all QUAT sanitizer container solution and refilled it at the dispenser.
      • However, it seems like the QUAT is dissipating too quickly when dispensed into the containers. Speak to your chemical represenstative to have them address issue.

      • Observation: One of the light shields still missing on a light fixture.
      • Corrective Action(s): As per Shift Manager, a replacement has been ordered for this fixture.

    • 205 - Cold potentially hazardous food stored/displayed above 4 °C. [s. 14(2)]
      • Observation (CORRECTED DURING INSPECTION): Cold lines (sliced tomatoes, cheeses and lettuce) were using time as a control however the time labels on the containers were incorrectly labeled. For example, sliced tomatoes only has a hold time of 4 hours at ambient temperature but the time label indicated was 4 PM when the inspection started at 11:15 AM.
      • Corrective Action(s): **Issue corrected at the time of inspection** New time labels were placed on the cold potentially hazardous items (time label for 2 PM was placed on the containers instead). As per the Assistant Manager, they are training a new staff and had forgotten to place the new time stamps on the containers. This is the second time this violation has been cited. Training is not an excuse as any food handlers working should be familiar with the Food Safety Plan prior to actually working.
      • **If this violation is ever noted again, this location may not be able to follow the procedures for using Time as a Control for any cold potentially hazardous food items anymore and this action will also lead to progressive enforcement actions. This is your last chance to ensure all food handlers must be familar with the procedures and follow all steps required.

      • Observation (CORRECTED DURING INSPECTION): 1. 0 - 100 PPM QUAT detected in all the QUAT sanitizer containers noted throughout the prep areas.
      • 2. Food debris noted on a produce slicer.
      • Corrective Action(s): **Issues corrected at the time of inspection**
      • 1. Assistant Manager refilled all sanitizer containers. 400 PPM QUAT detected after the buckets are refilled.
      • 2. Produce slicer was brought to the 3-compartment sink for washing. Please wash, rinse and sanitize all utensils/ equipment after use and inspect to ensure they are thoroughly cleaned before storing away.

      • Observation: 2 light fixtures noted with missing light shields in the drive-thru area.
      • Corrective Action(s): Replace the broken/ missing light shields.

      • 304 - Premises not free of pests [s. 26(a)]
      • Observation: Lots of fresh mouse droppings noted in two areas - behind the hot water tank and beside the syrup fountain boxes. Note: Immediately above both areas, the ceiling panels are missing.
      • Corrective Action(s): Clean, sweep and sanitize all areas noted with mouse droppings. This will discourage the mouse from coming back to this area as pests are attracted to their own fecal matters. Also, by cleaning the areas you will be able to monitor the situation if more droppings appear after the clean-up.
      • As per the Assistant Manager, their external pest company is coming twice a week to help address the current pest issue.

      • Corrective Action(s): Please seal all potential entry points for pests. This includes creating custom ceiling panels around the pipes and wires to seal off all entry points to avoid pest going in and out of the areas above the ceiling.

      • Observation (CORRECTED DURING INSPECTION): Noted sliced lettuce, tomatoes and cheese (all sliced) using time as a control and the time markings for each container was at 6 PM, current time of inspection was 11:30 AM.
      • Corrective Action(s): **Issue was identified to the Manager and corrected at the time of inspection**
      • As per Manager, a new food handler started working today and may be confused with using time as a control. The marked food items were discarded. Please follow the company's procedures for using time as a control.

      • Observation: Mouse droppings noted in the drive-thru area, on the bottom shelving unit and below the dry storage shelf across from the walk-in freezer.
      • Corrective Action(s): Droppings were noted in the same drive-thru area during the last routine inspection. If this was cleaned after the last inspection, then this is evident of recent activities. Manager contacted their external Pest Control Company to report issue during the inspection. Clean and sanitize all areas noted with pest droppings and continue to work with the Pest Control Company to resolve issue.
