      • Observation (CORRECTED DURING INSPECTION): Low temp dishwasher not achieving 50 to 100ppm Chlorine at plate level. The sanitizer is not pumping into the dishwasher.
      • Corrective Action(s): Use dishwasher to wash the dishes and then place the dishes into the 2 compartment sink for the sanitation step for 30 seconds at 100-200ppm Chlorine.
      • Call technician and have them repair the dishwasher in the meantime.

      • Observation (CORRECTED DURING INSPECTION): Bar handwashing sink lack paper towels.
      • Corrective Action(s): Ensure all handwashing sinks are well equipped with hot and cold running water, liquid soap, and paper towels at all times to ensure proper staff hygiene.

      • Observation: Rodent droppings observed behind chest freezer next to the hot water tank and walk-in cooler areas. Rodent droppings also observed underneath shelving units used to house toilettries.

      • Observation (CORRECTED DURING INSPECTION): Minced garlic probed at 16.9C, hydrated pad thai noodles probed at 10.1C. Both items were stored at room temperature over 2 hours ago.
      • Corrective Action(s): Minced garlic and hydrated pad thai noodles were discarded during the inspection. These foods must be stored in the cooler at 4C or colder when not in use. If required to use during meal rush - take out smaller portions and use up within 2 hours.

      • Observation: (1) Observed staff washing bean sprouts in the 2 compartment dishwashing sink.
      • (2) Observed staff washing dishes in the 2 compartment prep sink.
      • (3) Repeat violation. Can of open quail eggs and condensed milk observed in coolers.
      • (4) Spring rolls left uncovered in chest freezer.
      • (5) Rice paddles stored in room temperature water.
      • (6) Garbage bag used to cover inserts in the curry/soup prep cooler.
      • Corrective Action(s): (1) Discussion with manager regarding washing food in the dishwashing area . DO NOT wash food in 2 compartment dishwashing sink. This area is only for washing and sanitizing dishes. Remind all staff of proper food handling practices.

      • Corrective Action(s): Rice balls were probed to be -1C. Food was moved to walk in cooler during the inspection. Do not thaw food at room temperature. Thaw foods the night before in the walk in cooler.

      • Observation: (1) Grease accumulation observed on backsplash tiles behind the curry/soup cooking equipment.

      • (3) Some mice droppings observed on the floor the dry storage room. No droppings observed in other areas.
      • Corrective Action(s): (1 + 2) Thoroughly clean and sanitize area to prevent pest attraction.
      • (3) Pull out shelves and thoroughly clean and sanitize affected area. Monitor for pest activity daily after cleaning.

      • 2. Buildup noted in the interior of the ice machine.
      • Corrective Action(s): 1. **Issue corrected** Operator fixed the dish machine and 100 PPM chlorine residual was able to be detected at the plate level.
      • 2. Follow the manufacturer's instruction - Discard all ice. Clean and sanitize the interior of the ice machine. Maintain as per your Sanitation Plan after performing the deep cleaning.

      • Observation: 1. Leaking noted in the walk-in cooler from the fan unit.
      • 2. Walk-in cooler was noted with an internal temperature of 7.4C.
      • 3. Reach-in cooler in the back prep area (far back) was noted with an internal temperature of 11.2C.
      • 4. Low temperature dish machine in the kitchen area was detected with < 10 PPM chlorine residual at the start of the inspection.
      • Corrective Action(s): 1. Operator must have the walk-in cooler serviced so no water is leaking in the unit - this could potentially contaminate food stored nearby and become a tripping hazard as the floor was wet.
      • 2 & 3. Most food in these two malfunctioning coolers were whole vegetables or food not requiring refrigeration. Proteins stored in the walk-in cooler was defrosting and therefore the temperatures were within the appropriate ranges at around 4.0C. Other cold potentially hazardous food, the operator had placed on ice or will be transferring to another functioning cooler (capable of maintaining at 4.0C or below) after the lunch rush.
      • The cooler units were being serviced during the inspection.
      • 4. Operator fixed the low temperature dish machine and 100 PPM chlorine residual was detected at the plate level after adjusting.

      • Observation: 1. Improper dispensers (bowls with no handles) were used and stored within bulk condiment containers (e.g. sugar and salt).
      • 2. Opened canned food (quail eggs and coconut milk) noted stored in the walk-in cooler.
      • Corrective Action(s): 1. Remove all the bowls in the condiment containers and use appropriate ones with handles to prevent potential contamination of food when dispensing.
      • 2. Pour out all opened canned food into food grade containers if not used immediately within the same day.

      • Observation (CORRECTED DURING INSPECTION): 1. < 10 ppm chlorine residual detected in the bleach sanitizer spray bottle noted in the preparation area.
      • 2. < 10 ppm chlorine residual detected in the main low temperature dish machine. As per operator, dish machine was working fine as of yesterday night. Chlorine test strips available and they test the machine regularly for chlorine residual.
      • Corrective Action(s): **Issue corrected at the time of inspection**
      • 1. Discard the bleach sanitizer spray bottle and prepare another one with the following ratio - 2.5 tsp of bleach per 1 L water for 100 - 200 ppm chlorine residual mixture.
      • 2. Operator called their chemical representative during the inspection and was able to fix the machine during the inspection. 100 ppm chlorine residual was detected at the plate level after the rinse cycle.

      • Corrective Action(s): Correction Order issued. At this time, the dining area is closed. Facility may only serve take-out or to-go only until the dish machines are fixed and inspected by the area Health Inspector. All utensils and cookware are to be manually washed, rinsed and sanitized in the 2-compartment sink. Manual warewashing procedures reviewed with the operator.
